基于Gibbs反思循环模式的腹腔镜模拟培训在外科住院医师规范化培训中的应用研究

Application research on laparoscopic simulation training in surgical residents'standardized training based on Gibbs reflective cycle

目的探讨基于Gibbs反思循环模式的腹腔镜模拟培训在外科住院医师规范化培训中的应用及效果。方法选取在中国医科大学附属盛京医院普通外科进行第2年住院医师规范化培训的学员共40名,随机分成试验组(n=20)和对照组(n=20)进行培训;对照组采用传统教学方式进行腹腔镜模拟培训,试验组采用基于Gibbs反思循环的教学方式进行培训;分别于培训初始时、培训后第4周、第8周进行操作考核,同时对试验组进行满意度调查;分析两组学员的考核成绩以及试验组学员满意度调查结果。结果试验组与对照组相比,各项腹腔镜操作成绩均显著提高(P<0.05);而且随着时间的延长,两组的成绩差异日益显著(P<0.001);试验组住院医师各项满意度皆有提高,且学习时长未见明显延长。结论基于Gibbs反思循环模式的腹腔镜模拟培训可以有效地提高住院医师腹腔镜手术技能,提升住院医师的学习兴趣,缩短手术学习曲线,值得进一步推广。

Objective To explore the application value and effect of laparoscopic simulation training in surgical residents'standardized training based on Gibbs reflective cycle.Method 40 residents who were in the second year of standardized training in general surgery of Shengjing Hospital of China Medical University were selected and divided into the experimental group(n=20)and the control group(n=20).Traditional teaching mode was used in the laparoscopic simulation training of the control group,while the teaching mode based on Gibbs reflective cycle was adopted in the training of the experimental group.Skill operation was assessed at the begin-ning of the training,in the 4th week and the 8th week after training,and satisfaction survey was given to the experimental group.Assessment scores of the two groups and the results of satis-faction survey of the experimental group were analyzed.Results Compared with residents in the control group,the laparoscopic operation scores of the experimental group were significantly improved(P<0.05),and with the extension of time,the difference between the two groups was increasingly significant(P<0.001).Residents'satisfaction in the experimental group was im-proved,but time of study was not significantly prolonged.Conclusion Laparoscopic simulation training based on Gibbs reflective cycle can effectively improve the laparoscopic operation skills of residents in general surgery,enhance their learning interest,and shorten the learning curve,which is worth further promotion.
