大剂量胰岛素应用于钙离子通道阻滞剂中毒救治中的临床效果

Clinical Effect of High-Dose Insulin in the Treatment of Calcium Channel Blocker Poisoning

摘要 目的探讨大剂量胰岛素应用于钙离子通道阻滞剂(CCB)中毒血液净化救治中的临床效果。方法选取2015年1月至2021年1月盘锦市中心医院收治的79例早期CCB中毒患者作为研究对象,随机分为观察组40例接受大剂量胰岛素治疗,对照组39例接受大剂量钙剂救治,两组给予标准常规处理后再进行不同专科干预,治疗时间为4 d。比较两组住院、通气时间及并发症发生率,治疗前后循环指标(平均动脉压、心率、二氧化碳分压)。结果观察组通气时间、住院时间短于对照组(P<0.05);两组治疗前平均动脉压、心率、二氧化碳分压比较差异无统计学意义(P>0.05),治疗后观察组平均动脉压、心率高于对照组,二氧化碳分压低于对照组,差异有统计学意义(P<0.05);两组并发症发生率比较差异无统计学意义(χ^(2)=0.198,P>0.05)。结论大剂量胰岛素治疗CCB中毒效果显著,可有效改善患者循环指标,降低严重并发症发生,缩短康复时间。 Objective To explore the clinical effect of high-dose insulin used in the treatment of calcium channel blocker(CCB) poisoning blood purification.Methods A total of 79 patients with early CCB poisoning admitted to Panjin Central Hospital from January 2015 to January 2021 were selected as the research objects and were randomly divided into an observation group(40 cases,receiving high-dose insulin treatment)and a control group(39 cases,receiving high-dose calcium treatment).Both groups were given standard routine treatment and then undergoing different specialized interventions,and the treatment time was 4 days.The hospitalization,ventilation time,complication rate,and circulatory indicators(mean arterial pressure,heart rate,carbon dioxide partial pressure)before and after treatment were compared between the 2 groups.Results The ventilation time and hospitalization time of the observation group were shorter than those of the control group(P<0.05);There was no significant difference in mean arterial pressure,heart rate and partial pressure of carbon dioxide between the 2 groups before treatment(P>0.05).After treatment,the mean arterial pressure and heart rate of the observation group were higher than those of the control group,and the partial pressure of carbon dioxide was lower than that of the control group.There was no significant difference in the incidence of complications between the 2 groups(χ^(2)=0.198,P>0.05).Conclusion High-dose insulin is effective in treating patients with CCB poisoning,which can effectively improve the circulation of patients,reduce the occurrence of serious complications,and shorten the recovery time of patients.
