摘要
目的探究新型生物膜在疤痕子宫患者再次剖宫产术中的应用效果。方法回顾性分析2019年5月至2020年12月广州医科大学附属第四医院产科收治的71例疤痕子宫产妇的临床资料,按照是否使用新型生物膜将患者分为对照组40例(再次剖宫产未使用新型生物膜)和研究组31例(再次剖宫产使用新型生物膜)。随访6个月,比较术后6个月B超检测两组患者的子宫下段肌层最薄处厚度及术后患者满意度,术后6个月子宫异常出血及宫腔镜检查子宫疤痕缺陷情况。结果研究组患者术后6个月B超检测子宫下段肌层最薄处厚度为(8.57±0.73)mm,明显大于对照组的(6.21±0.85)mm,差异有统计学意义(P<0.05);研究组患者术后满意度为93.55%,明显高于对照组的70.00%,差异有统计学意义(P<0.05);术后6个月,宫腔镜检查发现,研究组患者异常子宫出血率为9.68%,明显低于对照组的30.00%,差异有统计学意义(P<0.05);研究组患者的子宫疤痕缺损率为25.81%,明显低于对照组的72.50%,差异有统计学意义(P<0.05)。结论疤痕子宫再次剖宫产术中应用新型生物膜能明显降低并发症的发生率,成为一种新型的安全、有效的方法,值得推广应用。
Objective To explore the clinical application of new biofilm in cesarean section in patients with scarred uterus.Methods The clinical data of 71 pregnant women with scarred uterus treated in Department of Obstetrics,the Fourth Affiliated Hospital of Guangzhou Medical University from May 2019 to December 2020 were retrospectively analyzed.According to whether to use the new biofilm,the patients were divided into the control group(40 cases,new biofilm was not used in the second cesarean section)and the study group(31 cases,new biofilm was used in the second cesarean section).The thickness of the thinnest part of the lower uterine myometrium and the satisfaction of patients in the two groups were measured by B-ultrasound 6 months after operation.The abnormal uterine bleeding and uterine scar defects were examined by hysteroscopy 6 months after operation.Results The thickness of the thinnest part of the lower uterine myometrium detected by B-ultrasound in the study group 6 months after operation was(8.57±0.73)mm,which was significantly higher than(6.21±0.85)mm in the control group(P<0.05).The postoperative satisfaction of patients in the study group was 93.55%,which was significantly higher than 70.00%in the control group(P<0.05).Six months after operation,hysteroscopy showed that the rate of abnormal uterine bleeding in the study group was 9.68%,which was significantly lower than 30.00%in the control group(P<0.05).The rate of uterine scar defect in the study group was 25.81%,which was significantly lower than 72.50%in the control group(P<0.05).Conclusion The application of new biofilm in cesarean section of scarred uterus is safe and effective,which can reduce the incidence of cesarean scar.It is worth popularizing.
