腹腔镜下小切口手术对胃十二指肠溃疡穿孔的治疗价值研究

Study on the Therapeutic Value of Laparoscopic Small Incision Surgery for Perforation of Gastroduodenal Ulcer

目的探讨胃十二指肠溃疡穿孔患者接受腹腔镜下小切口手术治疗的临床效果。方法从2018年3月—2020年12月期间来院接受治疗的胃十二指肠溃疡穿孔患者中随机抽取130例作为研究对象,以单双号分组的方式将其平均分为两组。研究组患者施以腹腔镜下小切口手术治疗,对照组患者施以常规手术治疗,比较治疗效果。结果研究组下床活动时间(14.56±3.54)h,手术时间(62.02±7.15)min,肛门排气时间(44.24±11.09)h,术中出血量(78.33±10.58)mL相较对照组显著更低,差异有统计学意义(t=9.534、14.543、5.776、16.725,P<0.05);研究组IL-6水平(2.04±0.37)pg/mL,IL-10水平(1.11±0.21)pg/mL,TFN-α水平(0.58±0.13)pg/mL均明显低于对照组,差异有统计学意义(t=11.490、24.627、6.456,P<0.05);治疗后研究组生理功能评分(93.88±2.46)分,精神状态评分(93.67±2.56)分,心理功能评分(94.10±2.73)分,总体健康评分(93.96±2.57)分均高于对照组,差异有统计学意义(t=12.639、12.542、13.543、12.627,P<0.05);研究组并发症发生率3.08%低于对照组15.38%,差异有统计学意义(χ^(2)=5.875,P<0.05)。结论将腹腔镜下小切口手术应用在胃十二指肠溃疡穿孔患者临床诊治中,可以显著提升诊疗效果,临床价值突出。

Objective To explore the clinical effect of laparoscopic small incision surgery in patients with gastroduodenal ulcer perforation.Methods 130 patients with gastroduodenal ulcer perforation who came to the hospital for treatment from March 2018 to December 2020 were randomly selected as the research object.They were divided into two groups evenly according to the odd and even numbers,and the patients in the study group were treated with laparoscopic small incision surgery.The patients in the control group were treated with conventional surgery to compare the treatment effects.Results The time of getting out of bed in the study group was(14.56±3.54)h,the operation time was(62.02±7.15)min,the time of anal exhaust was(44.24±11.09)h,and the intraoperative blood loss(78.33±10.58)mL was significantly lower than that of the control group,and the difference was statistically significant(t=9.534,14.543,5.776,16.725,P<0.05).IL-6 level(2.04±0.37)pg/mL,IL-10 level(1.11±0.21)pg/mL,and TFN-αlevel(0.58±0.13)pg/mL in the study group were significantly lower than those of the control group,and the difference was statistically significant(t=11.490,24.627,6.456,P<0.05).After treatment,the physical function score(93.88±2.46)points,the mental state score was(93.67±2.56)points,the mental function score(94.10±2.73)points,and the overall health score(93.96±2.57)points of the study group was higher than that of the control group,and the difference was statistically significant(t=12.639,12.542,13.543,12.627,P<0.05).The complication rate 3.08%of the study group was lower than that of the control group 15.38%,and the difference was statistically significant(χ^(2)=5.875,P<0.05).Conclusion The application of laparoscopic small incision surgery in the clinical diagnosis and treatment of patients with gastroduodenal ulcer perforation can significantly improve the diagnosis and treatment effect,has outstanding clinical value,and is suitable for promotion.
