分析以大叶性肺炎为临床表现的小儿肺炎支原体肺炎临床特点

Analysis on the Clinical Characteristics of Mycoplasma Pneumoniae Pneumonia in Children with Lobar Pneumonia as the Clinical Manifestation

目的分析以大叶性肺炎为临床表现的小儿肺炎支原体肺炎的临床表现及特点。方法选取2020年1月—2021年6月该院收治的300例肺炎支原体肺炎患儿为研究对象,根据发病原因不同分为大叶性肺炎、间质性肺炎、支气管肺炎3组,每组100例,回顾比较其各自的临床特点及差异。结果3组肺炎支原体肺炎患儿的临床症状、体征比较,差异有统计学意义(P<0.05)。大叶性肺炎组患儿的NEU%(4.00%)显著低于其他两组,差异有统计学意义(χ^(2)=17.411,P<0.05),而CRP(35.17±12.09)mg/L、LDH(388.28±125.38)U/L、ESR检测值(40.40±17.28)mm/h、心动过速(48.00%)、胸腔积液的检出率(45.00%)显著高于其他两组,差异有统计学意义(P<0.05)。且大叶性肺炎组糖皮质激素的使用率、吸氧率、转院外支气管镜灌洗率均显著高于其他两组,差异有统计学意义(P<0.05)。结论以大叶性肺炎为主要临床表现的支原体肺炎患儿有独特临床特点,掌握其临床特点有利于患儿的治疗。

Objective To analyze the clinical manifestations and characteristics of mycoplasma pneumoniae pneumonia in children with lobar pneumonia as clinical manifestations.Methods A total of 300 children with mycoplasma pneumoniae pneumonia admitted to the hospital from January 2020 to June 2021 were selected as the research objects.According to the different causes of disease,they were divided into three groups:lobar pneumonia,interstitial pneumonia,and bronchial pneumonia,with 100 cases in each group.Reviewed and compared their respective clinical characteristics and differences.Results The clinical symptoms and signs of children with mycoplasma pneumoniae pneumonia in the three groups were compared,and the differences were statistically significant(P<0.05).The NEU%(4.00%)of children in the lobar pneumonia group was significantly lower than that of the other two groups,and the difference was statistically significant(χ^(2)=17.411,P<0.05).The detection rate of CRP(35.17±12.09)mg/L,LDH(388.28±125.38)U/L,ESR detection value(40.40±17.28)mm/h,tachycardia(48.00%),and pleural effusion(45.00%)was significantly higher than the other two groups,and the difference was statistically significant(P<0.05).In addition,the glucocorticoid use rate,oxygen uptake rate,and bronchoscopy lavage rate outside the hospital in the lobar pneumonia group were significantly higher than the other two groups,the difference was statistically significant(P<0.05).Conclusion Children with mycoplasma pneumonia whose main clinical manifestations are lobar pneumonia have their unique clinical characteristics,and mastering their clinical characteristics is beneficial to the treatment of children.
