无创血流动力学监测在心力衰竭患者中的应用效果及对心率、收缩压的影响分析 被引量:1

摘要 目的探讨心力衰竭患者中无创血流动力学监测的应用效果及对心率、收缩压的影响。方法该文将2018年2月—2020年2月收治的心力衰竭患者60例作为研究对象,对患者实施个体化治疗,同时加强无创血流动力学监测,对患者治疗前后的收缩压(SBP)、舒张压(DBP)、心输出量(CO)、心脏指数(CI)、心率(HR)、每搏输出量(SV)、收缩时间比率(STR)、平均动脉压(MAP)、中心静脉压(CVP)、全心射血分数(GEF)、全心舒张末期容积指数(GEDI)进行对比,比较其脑钠肽(BNP)、左心射血分数(LVEF)、左心室收缩末期内径(LVESD)、左室舒张末期内径(LVEDD)及6 min步行距离变化。结果治疗前后血流动力学指标相比,治疗后患者的SBP、DBP、HR、CVP、STR明显高于治疗前,而MAP、CO、CI、GEF、SV、GEDI明显低于治疗前,差异有统计学意义(P<0.05);心功能指标相比,治疗后患者BNP较治疗前高,而LVEF、LVESD、LVEDD及6 min步行距离较治疗前低,差异有统计学意义(P<0.05)。结论通过无创血流动力学监测可将患者的血流动力学紊乱等变化情况准确且实时反映,有助于临床工作者及时发现患者的血管功能异常情况,为其制订针对性的治疗方案,具有较高的应用价值。 Objective To explore the application effect of non-invasive hemodynamic monitoring in patients with heart failure and its effect on heart rate and systolic blood pressure.Methods In this paper,60 patients with heart failure who were treated from February 2018 to February 2020 were taken as the research objects,and individualized treatment was given to the patients.At the same time,non-invasive hemodynamic monitoring was strengthened.Diastolic blood pressure(DBP),cardiac output(CO),cardiac index(CI),heart rate(HR),stroke volume(SV),systolic time ratio(STR),mean arterial pressure(MAP),central venous pressure(CVP),global ejection fraction(GEF),and global end-diastolic volume index(GEDI)were compared,and their brain natriuretic peptide(BNP),left ventricular ejection fraction(LVEF),and left ventricular end-systolic diameter(LVESD),left ventricular end-diastolic diameter(LVEDD)and changes in 6-min walking distance were compared.Results Compared with the hemodynamic indexes before and after treatment,the SBP,DBP,HR,CVP and STR of the patients after treatment were significantly higher than those before treatment,while MAP,CO,CI,GEF,SV and GEDI were significantly lower than those before treatment,the difference was statistically significant(P<0.05);compared with cardiac function indexes,BNP of patients after treatment was higher than that before treatment,while LVEF,LVESD,LVEDD and 6-min walking distance were lower than those before treatment,the difference was statistically significant(P<0.05).Conclusion Non-invasive hemodynamic monitoring can accurately and real-time reflect changes in patients'hemodynamic disorders,which is helpful for clinicians to detect abnormal vascular function of patients in time and formulate targeted treatment plans for them,which has high application value.
