基于SHEL模型的Check list表联合护理角色分工在先天性心脏病患儿术后转入交接管理中的应用

Application of Check list table based on SHEL model combined with nursing role division in postoperative transfer management of children with congenital heart disease

目的探讨基于SHEL模型的Check list表联合护理角色分工在先天性心脏病患儿术后转入交接管理中的应用效果。方法选择2020年4月1日至2021年8月31日苏州大学附属儿童医院新调入心胸外科监护室17名护士作为研究对象,根据猜硬币的方法将研究对象分为对照组、观察组,正面为对照组(8名),反面为观察组(9名)。对照组采用传统流程进行床单元、仪器设备、物品准备及转入后交接;观察组使用基于SHEL模型的Check list表单联合护理角色分工进行床单元、仪器设备、物品准备及转入后交接,并责任分工。比较两组的床单元准备、仪器设备准备、物品准备时间及正确率,转入交接时间,交接内容遗漏率。结果观察组进行床单元、仪器设备、交接物品准备时间短于对照组,准备正确率高于对照组,术后转入交接使用时间短于对照组,交接内容遗漏率低于对照组,差异均有统计学意义(P<0.05)。结论以SHEL模型为指导的Check list表的制定,联合护理角色分工为护理人员提供全面指导,可有效提高护理人员工作效率,减少安全隐患。

Objective To explore the application effect of the Check list table based on the SHEL model combined with the division of nursing roles in the management of postoperative transfer in children with congenital heart disease.Methods The 17 nurses newly transferred to the Cardiothoracic Surgery Intensive Care Unit,Children’s Hospital of Soochow University from April 1,2020 to August 31,2021 were selected as the research subjects.According to the method of guessing coins,the research subjects were divided into the control group and the observation group,the positive side as the control group and the reverse side as the observation group.8 cases in the control group,9 cases in the observation group.In the control group,the traditional process was used to prepare bed units,equipment and items,and handover after transfer;the observation group used the Check list table based on the SHEL model combined with the division of nursing roles to prepare bed units,equipment and items,and handover after the transfer,and divided the responsibilities.The time and accuracy of bed unit preparation,equipment preparation,and item preparation,the time used for transferring,and the omission rate of handover content between the two groups were compared.Results The preparation time of bed units,equipment and handover items in the observation group was shorter than that in the control group,the accuracy of preparation was higher than that in the control group,the use time of postoperative handover was shorter than that in the control group,and the omission rate of the handover content was lower than that in the control group,the differences were statistically significant(P<0.05).Conclusion The formulation of Check list under the guidance of SHEL model combined with the joint division of nursing roles could provide comprehensive guidance for nursing staff,effectively improve the work efficiency of nursing staff,reduce safety risks.
