糖尿病肾病患者血液透析治疗中低血糖反应及其护理效果分析

Analysis of Hypoglycemia Reaction and Nursing Effect in Hemodialysis Treatment of Diabetic Nephropathy Patients

目的分析糖尿病肾病行血液透析治疗期间低血糖反应与护理措施。方法选择该院2018年8月—2021年8月82例糖尿病肾病行血液透析治疗患者。随机数字表分组,A组41例所选用的是综合性护理干预,B组41例采用常规化护理干预。对比两组透析后不同时间段的血糖变化、低血糖与其他不良反应概率、自护能力变化和护理满意度、疾病不确定感评分。结果A组患者在血液透析2、3、4 h后的血糖水平均高于B组,差异有统计学意义(P<0.05)。A组患者的低血糖发生率低于B组,除血压异常外,其他不良反应概率低于B组,差异有统计学意义(P<0.05)。护理前,两组患者的自护能力评分差异无统计学意义(P>0.05)。护理2周后,A组患者的自护能力评分高于B组,差异有统计学意义(P<0.05)。A组患者的护理满意度评分均高于B组,差异有统计学意义(P<0.05)。护理前,两组患者的疾病不确定感评分差异无统计学意义(P>0.05)。护理2周后,A组患者的疾病不确定感评分低于B组,差异有统计学意义(P<0.05)。结论为糖尿病肾病行血液透析治疗期间加用综合性护理干预可以减少低血糖情况,且能控制其他不良反应,提升患者的自护能力和满意度,消除其对于疾病的不确定感。

Objective To analyze hypoglycemia reaction and nursing measures during hemodialysis treatment of diabetic nephropathy.Methods From August 2018 to August 2021 a total of 82 patients with diabetic nephropathy were treated with hemodialysis.Random number table grouping,41 cases in group A were selected for comprehensive nursing intervention,and 41 cases in group B were selected for routine nursing intervention.The changes in blood glucose in different time periods after dialysis,the probability of hypoglycemia and other adverse reactions,the changes in self-care ability,nursing satisfaction and illness uncertainty score were compared between the two groups.Results The blood glucose levels of patients in group A were higher than those in group B after 2 h,3 h and 4 h of hemodialysis,and the difference was statistically significant(P<0.05).The incidence of hypoglycemia in group A was lower than that in group B,and except for abnormal blood pressure,the probability of other adverse reactions was lower than that in group B,and the difference was statistically significant(P<0.05).Before nursing,there was no statistically significant difference in the scores of self-care ability between the two groups(P>0.05).After 2 weeks of nursing,the self-care ability score of group A was higher than that of group B,and the difference was statistically significant(P<0.05).The nursing satisfaction scores of patients in group A were higher than those in group B,and the difference was statistically significant(P<0.05).Before nursing,there was no statistically significant difference in the disease uncertainty score between the two groups(P>0.05).After 2 weeks of nursing,the disease uncertainty score of group A was lower than that of group B,and the difference was statistically significant(P<0.05).Conclusion The addition of comprehensive nursing intervention during hemodialysis treatment for diabetic nephropathy can reduce hypoglycemia,control other adverse reactions,improve patients'self-care ability and satisfaction,and eliminate their uncertainty about the disease.
