口服益生菌对2型糖尿病患者血糖控制的影响 被引量:2

摘要 目的了解口服益生菌对新确诊2型糖尿病患者血糖控制的影响,探索控制2型糖尿病患者血糖的辅助方案。方法选取该院2020年1月—2021年3月新确诊的2型糖尿病患者160例为研究对象,初期控制血糖方案均为“三短一长(赖脯胰岛素联合甘精胰岛素方案)”,确定胰岛素治疗方案后将入组患者随机分为双歧杆菌乳杆菌三联活菌片组(益生菌组)和非双歧杆菌乳杆菌三联活菌片组(非益生菌组),益生菌组予胰岛素治疗的同时规律服用双歧杆菌乳杆菌三联活菌片,非益生菌组仅给予胰岛素治疗,在治疗前及治疗3个月后分别对两组患者糖代谢及脂代谢情况进行评估,比较口服双歧杆菌乳杆菌三联活菌片对初治2型糖尿病患者血糖代谢、脂代谢的影响。结果益生菌组和非益生菌组患者的年龄、性别构成,差异无统计学意义(P>0.05),治疗前两组患者糖化血红蛋白、空腹血糖、餐后2 h血糖、胰岛素抵抗指数、平均动脉压、体质指数、腰臀比、三酰甘油、总胆固醇、低密度脂蛋白胆固醇、高密度脂蛋白胆固醇均差异无统计学意义(P>0.05)。治疗3个月后,两组患者糖代谢均较前改善,脂代谢改善不明显,其中益生菌组糖化血红蛋白、空腹血糖、餐后2 h血糖较非益生菌组改善更明显,差异有统计学意义(P<0.05)。结论口服益生菌联合胰岛素治疗更有助于改善2型糖尿病患者糖代谢,可作为2型糖尿病患者血糖控制的辅助方案。 Objective To understand the effect of oral probiotics on blood glucose control in newly diagnosed type 2 diabetic patients,and to explore auxiliary programs to control blood glucose in type 2 diabetic patients.Methods Selected 160 newly diagnosed type 2 diabetes patients in the hospital from January 2020 to March 2021.The initial blood glucose control program was"three short and one long(insulin lispro combined insulin glargine program)".After determining the insulin treatment plan,the enrolled patients were randomly divided into the Bifidobacterium Lactobacillus triple viable group(probiotic group)and the non-Bifidobacterium Lactobacillus triple viable group(non-probiotic group).The probiotics group was given insulin treatment and regularly taken Bifidobacterium lactobacillus triple live bacteria tablets.The non-probiotic group was given insulin therapy only,and the glucose metabolism and lipid metabolism of the two groups of patients were evaluated before the start of treatment and after 3 months of treatment.Compared the effects of oral administration of Bifidobacterium and Lactobacillus triple live bacteria tablets on blood glucose metabolism and lipid metabolism in newly treated patients with type 2 diabetes.Results There was no statistically significant difference in age and gender composition between the probiotic group and the non-probiotic group(P>0.05).Before treatment,the two groups of patients had no glycosylated hemoglobin,fasting blood glucose,2 h postprandial blood glucose,insulin resistance index,mean arterial pressure,body mass index,waist-to-hip ratio,triglycerides,total cholesterol,low-density lipoprotein cholesterol,and high-density lipoprotein cholesterol were compared,the difference was not statistically significant(P>0.05).After 3 months of treatment,the glucose metabolism of the two groups of patients improved compared with before,but the improvement of lipid metabolism was not obvious.Among them,glycosylated hemoglobin,fasting blood glucose,and blood glucose 2 h after a meal in the probiotic group were more significantly improved than those in the non-probiotic group, the difference was statistically significant (P<0.05). Conclusion Oral probiotics combined with insulin therapy is more helpful to improve glucose metabolism in patients with type 2 diabetes, and can be used as an auxiliary program for blood glucose control in patients with type 2 diabetes.
