多囊卵巢综合征伴不孕症育龄患者生命质量调查及影响因素分析

Investigation of the life quality of infertility women of childbearing age with polycystic ovary syndrome and its influencing factors analysis

目的:调查多囊卵巢综合征(PCOS)伴不孕症育龄患者的生命质量并分析相关影响因素。方法:抽取2020年3月-2021年3月在本院就诊的PCOS不孕症患者254例(PCOS组)和200名健康女性(对照组),发放生命质量调查表,调查对比两组生命质量调查结果,分析影响PCOS患者生命质量评分的相关因素。结果:PCOS组生理健康总分(70.47±15.52分)、心理健康总分(63.35±18.14分)及生命质量总分(65.64±16.41分)均低于对照组(80.34±16.95分、78.23±12.48分、79.10±12.25分)(P<0.05)。不同年龄、文化程度、职业、家庭月收入,以及有无既往病史、家族史、痛经、痤疮、溢脂的PCOS患者生命质量总分均无差异(均P>0.05),不同体质指数、PCOS患病年限、月经紊乱年限患者的生命质量总分有差异(P<0.05)。多元线性回归分析显示,体质指数>28kg/m^(2)、PCOS患病>5年、月经紊乱>2年、有多毛症、有黑棘皮均是影响PCOS不孕症患者生命质量的主要因素(均P<0.05)。结论:PCOS不孕症患者的生命质量水平低于健康女性,PCOS患病年限长、高体质指数、多毛、黑棘皮症、月经紊乱年限长均是影响其生命质量的主要因素,临床应加以心理引导。

Objective: To investigate the quality of life of women of childbearing age with polycystic ovary syndrome(PCOS), and to analyze its related influencing factors. Methods: 254 infertility women with PCOS were selected in observation group and 200 healthy women were selected in control group from March 2020 to March 2021. The questionnaire of quality of life was distributed to the women in the two groups. The survey results of quality of life of the women were compared between the two groups. The related factors affecting the quality of life of the infertility women with PCOS were analyzed. Results: The physical health score(70.47±15.52 points), the mental health score(63.35±18.14 points), and the total score of quality of life(65.64±16.41 points) of the women in the observation group were significantly lower than those(80.34±16.95 points, 78.23±12.48 points, and 79.10±12.25 points) of the women in the control group(P<0.05). There was no significant difference in the total score of life quality of the infertility women with PCOS among the women with different age, among the women with different education level, among the women with different occupation, or among the women with different monthly family income, or between the women with and without past medical history, between the women with and without family history, between the women with and without dysmenorrhea, between the women with and without acne, or between the women with and without seborrheic dermatitis(all P<0.05), but there was significant difference in the total score of life quality of the infertility women with PCOS among the women with different body mass index, among the women with different duration of PCOS disease years, or among the women with different duration of menstrual disorder(P<0.05). Multiple linear regression analysis showed that the body mass index >28kg/m^(2),the duration of PCOS >5years,the duration of menstrual disorder>2years,the hypertrichosis,the acanthosis nigricans of the infertility women with PCOS were the main factors affecting their quality of life(all P<0.05).Conclusion:The quality of life of the infertile women with PCOS is lower than that of the healthy women.The longer duration of PCOS,the higher value of body mass index,the hypertrichosis,the acanthosis nigricans,and the longer duration of menstrual disorder of the infertility women with PCOS are the main factors affecting their quality of life,so the psychological guidance should be conducted for the infertility women with PCOS in clinic.
