IGF-1、IL-18、TNF-α水平与妊娠期亚临床甲减相关性

Correlation between the insulin-like growth factor Ⅰ,interleukin-18,and tumor necrosis factor-αlevels of pregnant women and their subclinical hypothyroidism occurrence

摘要 目的:探讨IGF-1、IL-18、TNF-α水平与妊娠期亚临床甲减(SCH)关系。方法:选择2019年1月-2020年12月本院治疗的妊娠期亚临床甲减孕妇123例为SCH组,健康孕妇153例为对照组。比较两组血清胰岛素样生长因子Ⅰ(IGF-1)、白介素-18(IL-18)、肿瘤坏死因子-α(TNF-α)水平。结果:促甲状腺激素(TSH)水平SCH组高于对照组。在妊娠早中晚期,与对照组相比,SCH组IGF-1较低而IL-18、TNF-α水平较高(均P<0.01);IGF-1水平与SCH发病呈负相关,而IL-18、TNF-α水平与SCH发病呈正相关。多重线性回归分析指出,早孕期IGF1(Beta=-0.463,P=0.003)与TSH水平呈负相关,而IL-18(Beta=-0.144,P=0.007)、TNF-α(Beta=0.133,P=0.009)与TSH水平呈正相关。结论:IGF-1、IL-18、TNF-α与SCH的发病以及TSH水平有一定关系,提示可作为临床检测指标。 Objective:To investigate the correlation between the insulin-like growth factor Ⅰ(IGF-Ⅰ),interleukin-18(IL-18),and tumor necrosis factor-α(TNF-α)levels of pregnant women and their subclinical hypothyroidism(SCH)occurrence.Methods:123pregnant women with SCH were selected in study group and 153healthy pregnant women were selected in control group between January 2019and December 2020.The levels of serum IGF-1,IL-18,and TNF-αof the women were compared between the two groups.Results:The level of thyroid stimulating hormone(TSH)of the women in the study group was significantly higher than that of the women in the control group.During the first,the second,and the third trimester of pregnancy,the IGF-1level of the women in the study group was significantly lower than that of the women in the control group,while the levels of IL-18and TNF-αof the women in the study group were significantly higher than those of the women in the control group(all P<0.01).The IGF-1level of the women was negatively correlated with their SCH occurrence,while the levels of IL-18and TNF-αof the women were positively correlated with their SCH occurrence.Multiple linear regression analysis showed that during the first trimester of pregnancy,the IGF1level of the women was negatively correlated with their TSH level(Beta=-0.463,P=0.003),while the IL-18level(Beta=-0.144,P=0.007)and the TNF-αlevel(Beta=0.133,P=0.009)of the women were positively correlated with their TSH level.Conclusions:The levels of IGF-1,IL-18,and TNF-αof the pregnant women are all correlate with their TSH level,which maybe as the detection indexes of SCH in clinic.
