子宫内膜异位症术后戈舍瑞林联合桂枝茯苓胶囊治疗效果及对患者炎症因子水平影响

Effect of goserelin combined with Guizhi fuling capsule for treating women with endometriosis after operation and its influence on the level of their inflammatory factors

目的:观察子宫内膜异位症(EMT)术后戈舍瑞林联合桂枝茯苓胶囊疗效及患者血清前列腺素(PG)E_(2)、PG F2α、血栓素(TXB2)水平变化。方法:采取前瞻性研究,以2018年9月-2019年4月在本院诊断并治疗的EMT患者88例,按照随机分组原则分为观察组和对照组各44例,两组均行EMT手术且术后采用戈舍瑞林治疗,观察组加用桂枝茯苓胶囊,共治疗3个月。比较治疗效果、性激素水平、PGE_(2)、PGF2α、TXB2水平以及不良反应。结果:治疗后两组分级评分及疼痛评分均下降且观察组低于对照组,两组雌二醇、促黄体生成素、卵泡刺激素、睾酮、性结合球蛋白均显著改善且观察组优于对照组,两组PGE_(2)、PGF2α、TXB2水平均下降且观察组低于对照组,术后随访1年观察组复发率(6.8%)低于对照组(22.7%)(均P>0.05);两组发生恶心呕吐、乳房肿胀、阴道出血以及潮热等不良反应无差异(P>0.05)。结论:EMT术后采用戈舍瑞林联合桂枝茯苓胶囊治疗,可提高手术效果,降低复发,下调患者血清炎性因子水平,用药安全性较好。

Objective:To observe the curative effect of goserelin combined with Guizhi fuling capsule for treating patients with endometriosis(EMT)after operation,and to study the changes of serum PG E_(2)and PG F2α,and TXB2 levels of these patients.Methods:A prospective study was conducted in 88 patients with EMT from September 2018 to April 2019.These patients were randomly divided into observation group and control group(44 cases in each group).The patients in both groups were treated with goserelin after operation for 3 months,and the patients in the observation group were treated with Guizhi fuling capsule additionally for 3 months.The therapeutic effect,the levels of sex hormone,PGE_(2),PGF2α,and TXB2,and the adverse reactions rate of the patients were compared between the two groups.Results:After treatment,the grading score of EMT and the pain score of the patients in the two groups had decreased significantly,and which of the patients in the observation group were significantly lower than those the patients in the control group.The levels of estradiol,luteinizing hormone,follicle stimulating hormone,testosterone,and sex binding globulin of the patients in the two groups had improved significantly after treatment,and which of the patients in the observation group were significantly better than those the patients in the control group.The levels of PGE_(2),PGF2α,and TXB2 of the patients in the two groups had decreased significantly after treatment,and which of the patients in the observation group were significantly lower than those of the patients in the control group.The recurrence rate(6.8%)of the patients in the observation group during postoperative followed up for 1 year was significantly lower than that(22.7%)of the patients in the control group(all P>0.05).There were no significant differences in the rates of adverse reactions,such as nausea and vomiting,breast swelling,vaginal bleeding,and hot flashes,of the patients between the two groups(P>0.05).Conclusion:Goserelin combined with Guizhi fuling capsule for treating women with EMT after operation can improve their surgical effect,reduce the recurrence of EMT,down-regulate the level of their serum inflammatory factor with better drug safety.
