大理地区单中心314例泌尿系结石成分分析 被引量:3

Analysis of composition of the urinary calculi in 314 cases of single center in Dali

摘要 目的探讨大理地区泌尿系结石患者的结石分布情况及成分特点,为该地区泌尿系结石的临床防治提供依据。方法收集2017年1月至2018年1月本院收治的314例泌尿系结石患者的临床资料,应用红外光谱仪进行结石成分分析。结果314例泌尿系结石患者中,50~59岁是高发年龄段,占总发病人群的30.57%(96/314)。男女性别比例为1.88∶1。上、下尿路结石均以含草酸钙结石最高,但上、下尿路结石成分比较,差异无统计学意义(P>0.05)。草酸钙结石检出286例,其中混合结石177例,以一水草酸钙+二水草酸钙+碳酸磷灰石为主,占24.84%;单一结石137例,其中以一水草酸钙为主,占35.35%。白族和汉族的结石主要成分比较,差异无统计学意义(P>0.05)。结论大理地区的泌尿系结石主要由草酸钙结石、感染性结石以及尿酸盐组成。该地区的泌尿系结石成分复杂,只有完成结石成分分析才能为患者指导个性化预防措施。 Objective To explore the distribution and composition of urinary calculi in Dali,and to provide evidence for clinical prevention and treatment of urinary calculi.Methods From January 2017 to January 2018,clinical data of 314 patients with urinary calculi admitted were collected,and the composition of urinary calculi were analyzed by infrared spectrum.Results Among the 314 patients with urinary calculi,The age group of 50-59 years was the most common,accounting for 30.57%(96/314)of the total cases.The male to female ratio was 1.88∶1.Calcium oxalate was the highest in upper and lower urinary tract stones,but there was no statistical significance in composition of upper and lower urinary tract stones(P>0.05).Calcium oxalate calculus was detected in 286 cases,including 177 cases of mixed calculus,mainly calcium oxalate monohydrate with calcium oxalate dihydrate with carbonate apatite,accounting for 24.84%.Single calculi were found in 137 cases,mainly calcium oxalate monohydrate,accounting for 35.35%.There was no significant difference in stone components between Bai nationality and Han nationality(P>0.05).Conclusions Urinary calculi in Dali area mainly consist of calcium oxalate calculi,infectious calculi and urate.Urolithiasis composition in Dali area is complex,only complete calculus composition analysis can guide personalized preventive measures for patients.
