不同乙肝五项模式乙型肝炎患者血清HBV-DNA检测结果及其作用

目的探讨不同乙肝五项模式乙型肝炎患者血清乙肝病毒脱氧核糖核酸(HBV-DNA)检测结果及其作用。方法选取2019年10月至2020年10月本院收治的90例乙型肝炎患者作为研究对象,并根据HBeAg是否表达分为HBeAg表达组(n=34)和HBeAg未表达组(n=56)。采用酶联免疫吸附法(ELISA)测定乙肝五项,采用荧光定量聚合酶链式反应(FQ-PCR)法检测血清HBV-DNA,比较不同乙肝五项模式乙型肝炎患者HBV-DNA阳性检出率,分析血清HBV-DNA浓度与乙型肝炎e抗原(HBeAg)检出率及HBeAg表达与血清HBV-DNA阳性检出率的关系。结果乙型肝炎大三阳患者的HBV-DNA阳性检出率高于乙型肝炎小三阳、小二阳患者(P<0.05)。血清HBV-DNA浓度>10^(7)U/ml的HBeAg检出率明显高于其他血清HBV-DNA浓度(P<0.05);血清HBV-DNA浓度与HBeAg检出率呈正相关(r=0.449,P<0.05)。HBeAg表达组血清HBV-DNA阳性检出率为94.1%,显著高于HBeAg未表达组的21.4%(P<0.05);HBeAg表达与血清HBV-DNA阳性检出率呈正相关(r=0.482,P<0.05)。结论对乙型肝炎患者应进行乙肝五项检查联合血清HBV-DNA检测,以提高诊断准确率和判断病情严重度,为临床治疗提供依据。

Objective To investigate the detection results and effects of serum hepatitis B virus deoxyribonucleic acid(HBV-DNA)in hepatitis B patients with different hepatitis B five patterns.Methods 90 patients with hepatitis B treated in our hospital from October 2019 to October 2020were selected as the research subjects,and they were divided into HBeAg expression group(n=34)and HBeAg non-expression group(n=56)according to whether HBeAg was expressed or not.Five items of hepatitis B were detected by enzyme-linked immunosorbent assay(ELISA),and the serum HBV-DNA was detected by fluorescence quantitative polymerase chain reaction(FQ-PCR),the positive rate of HBV-DNA in different hepatitis B patients were compared,the relationship between serum HBV-DNA concentration and the detection rate of hepatitis B e antigen(HBeAg)and the relationship between HBeAg expression and serum HBV-DNA positive detection rate were analyzed.Results The positive rate of HBV-DNA in patients with big three yang of hepatitis B was higher than that in patients with small three yang and small two yang of hepatitis B(P<0.05).The detection rate of HBeAg with serum HBV-DNA concentration>10^(7) U/mL was significantly higher than other serum HBV-DNA concentrations(P<0.05);the concentration of serum HBV-DNA was positively correlated with the detection rate of HBeAg(r=0.449,P<0.05).The positive rate of serum HBV-DNA in HBeAg expression group was 94.1%,which wassignificantly higher than 21.4%of HBeAg non expression group;the expression of HBeAg was positively correlated with the positive detection rate of serum HBV-DNA(r=0.482,P<0.05).Conclusion Five items of hepatitis B examination combined with HBV-DNA detection should be carried out in patients with hepatitis B to improve the accuracy of diagnosis and determine the severity of the disease,so as to provide evidence for clinical treatment.
