慢性萎缩性胃炎致病因素的中西医研究进展

Research progress on pathogenic factors of chronic atrophic gastritis in Traditional Chinese and Western Medicine

慢性萎缩性胃炎(CAG)作为一种常见癌前疾病,临床检出率与复发率逐年增高,受到国内外医学界的广泛关注。本病从多方面影响着患者的生活质量,发病机制复杂且不明确,发病过程涉及多种致病因素,明确其致病因素,对于临床风险因素识别和疾病监测有重要意义。本文从中西医两方面对CAG的致病因素作一综述,西医致病因素包括微生物感染、自身免疫、胆汁反流、宿主因素、家族遗传、饮食习惯、营养素摄入、长期吸烟、缺乏运动、负面情绪、职业习惯、地域住所、药物毒性;中医致病因素包括外邪因素、饮食因素、情志因素、素体因素、体质因素。已有研究多是探究单一类别致病因素对CAG的影响,缺少全面性总结,文章对以上中西医致病因素作出总结,并分别阐述其具体影响,旨在帮助临床工作者早期识别并干预CAG进展性变化,降低临床胃癌发生率。

Chronic atrophic gastritis(CAG) is a common precancerous disease, and its clinical detection rate and recurrence rate are increasing year by year. The disease affects the quality of life of patients in many ways. The pathogenesis is complex and unclear. The pathogenesis involves a variety of pathogenic factors. Identifying the pathogenic factors is of great significance for the identification of clinical risk factors and disease monitoring. This article reviews the pathogenic factors of CAG from both Traditional Chinese and Western Medicine. The pathogenic factors of western medicine include microbial infection, autoimmunity, bile reflux, host factors, family inheritance, dietary habits, nutrient intake, long-term smoking, lack of exercise, negative emotions, occupational habits, regional residence and drug toxicity;TCM pathogenic factors include exogenous factors, dietary factors, emotional factors and physical factors. Existing studies mostly explore the influence of a single type of pathogenic factors on CAG, while lack a comprehensive summary. This article summarizes the above pathogenic factors in Traditional Chinese and Western Medicine, and expounds their specific effects, aiming to help clinicians identify and intervene early. Progressive changes in CAG reduce the incidence of clinical gastric cancer.
