基于《中国高血压防治指南(2018修订版)》对某院门诊降压药应用情况的调查

Investigation of Application of Antihypertensive Drugs in Outpatient Department of a Hospital Based on Chinese Guideline for the Management of Hypertension(Revised in 2018)

目的:结合《中国高血压防治指南(2018修订版)》调查常熟市第二人民医院门诊降压药应用情况,旨在提高降压药物应用规范性与合理性。方法:回顾性抽查2019年1-6月常熟市第二人民医院门诊高血压病历处方共1768张,统计患者基本资料、用药品种及联合方案,分析其规范性、合理性。结果:高血压药物应用最广泛的为钙通道阻滞剂和血管紧张素受体拮抗剂。治疗方案中单药治疗处方占44.45%,其余均采用了降压药物联合应用,联合治疗方案与《中国高血压防治指南(2018修订版)》推荐基本一致。处方用药合理率99.10%,个别处方存在用法用量不合理、选药不适宜情况。结论:门诊处方中降压药应用与《中国高血压防治指南(2018修订版)》基本相符,用药基本合理,但仍建议临床医师对指南及相关药物(如复方制剂),应用的合理性继续加以注意,以保证患者用药安全有效。

Objective:To investigate the application of antihypertensive drugs in outpatient department of a hospital according to the Chinese Guideline for the Management of Hypertension(Revised in 2018),and to improve the standardization and rationality of antihypertensive drug application.Methods:A total of 1768 prescriptions for outpatient hypertension cases in Changshu No.2 People's Hospital from January to June in 2019 were retrospectively checked.The basic data of the patients,the types of medications and the combination regimen were counted to analyze their standardization and rationality.Results:The most widely used antihypertensive drugs were calcium channel blockers and angiotensin receptor antagonists.In the treatment regimen,singledrug prescriptions accounted for 44.46%,and the rest were the combined application of antihypertensive drugs,the combined treatment regimen was basically the same as the recommendation in Chinese Guideline for the Management of Hypertension(Revised in 2018).The qualification rate of drug use in the prescriptions was 99.10%,and some prescriptions had unreasonable usage or dosage,and inappropriate drug selection.Conclusion:The application of antihypertensive drugs in outpatient prescriptions is basically consistent with the Chinese Guideline for the Management of Hypertension(Revised in 2018),and the medications are basically reasonable.However,it is recommended that clinicians pay more attention to the rationality of the guidelines and related drugs such as compound preparations to ensure the safe and effective medication of patients.
