硒酵母联合左甲状腺素钠片治疗桥本甲状腺炎合并甲状腺功能减退症临床效果观察

Clinical Effect Observation of Selenium Yeast Combined with Levothyroxine Sodium Tablets in Treatment of Hashimoto's Thyroiditis and Hypothyroidism

目的:分析在桥本甲状腺炎合并甲状腺功能减退症治疗过程中应用硒酵母联合左甲状腺素钠片的疗效。方法:将2018年7月-2019年7月北京市朝阳区劲松社区卫生服务中心收治的50例桥本甲状腺炎合并甲状腺功能减退症患者作为研究对象,按照双盲法随机分为两组,各25例。对照组应用左甲状腺素钠片治疗,研究组应用硒酵母联合左甲状腺素钠片治疗。对比两组患者血硒水平、甲状腺功能、甲状腺自身抗体水平以及不良反应发生情况。结果:治疗后,研究组血硒水平、血清游离甲状腺素、游离三碘甲状腺原氨酸水平高于对照组,促甲状腺激素水平低于对照组,差异均具有统计学意义(P<0.05);治疗后,研究组抗甲状腺球酶蛋白抗体、甲状腺过氧化物酶抗体水平低于对照组,差异具有统计学意义(P<0.05);两组患者不良反应发生率比较,差异无统计学意义(P>0.05)。结论:硒酵母联合左甲状腺素钠片治疗桥本甲状腺炎合并甲状腺功能减退症,能显著改善甲状腺功能,临床治疗效果较好,且具有较高的安全性。

Objective:To analyze the efficacy of selenium yeast combined with levothyroxine sodium tablets in the treatment of Hashimoto's thyroiditis and hypothyroidism.Methods:Fifty patients with Hashimoto's thyroiditis and hypothyroidism were selected from Jinsong Community Health Service Center in Chaoyang District of Beijing from July 2018 to July 2019,as the study subjects.They were randomly divided into two groups according to the double-blind method,with 25 cases in each group.The control group was treated with levothyroxine sodium tablets,and the study group was treated with selenium yeast combined with levothyroxine sodium tablets.The blood selenium levels,thyroid function,thyroid autoantibody levels,and adverse reactions were compared between the two groups of patients.Results:After treatment,the levels of blood selenium,serum free thyroxine and free triiodothyronine in the study group were higher than those in the control group,and the level of thyroid-stimulating hormone was lower than that in the control group,and the differences were statistically significant(P<0.05).After treatment,the levels of antithyroglobulin antibody and thyroid peroxidase antibody in the study group were lower than those in the control group,and the difference was statistically significant(P<0.05);there was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion:The combination therapy of selenium yeast and levothyroxine sodium tablets in patients with Hashimoto's thyroiditis and hypothyroidism can significantly improve the thyroid function,achieve better clinical treatment effect and has high safety.
