摘要
目的:分析氧疗联合肺康复功能锻炼对慢性阻塞性肺疾病(COPD)合并肺心病的疗效。方法:选取2019年6月-2020年6月敦煌市七里镇青海油田医院收治的COPD合并肺心病患者100例为研究对象,随机分为对照组和观察组,各50例。对照组采用肺康复功能锻炼;观察组患者在肺康复功能锻炼基础上采用氧疗治疗。比较两组患者临床疗效,治疗2周后氧分压(PaO_(2))、二氧化碳分压(PaCO_(2))、第1秒用力呼气容积(FEV_(1))、FEV_(1)/用力肺活量(FVC)水平,临床症状改善时间、圣乔治呼吸问卷评分(SGRQ)及肺动脉收缩压、心脏Tei指数的改善情况。结果:观察组治疗总有效率高于对照组,治疗2周后PaO_(2)、FEV_(1)、FEV_(1)/FVC水平高于对照组,PaCO_(2)水平低于对照组,临床症状改善时间低于对照组,SGRQ评分、肺动脉收缩压、心脏Tei指数低于对照组,以上差异均有统计学意义(P<0.05)。结论:对COPD合并肺心病患者采用长期氧疗联合肺康复功能锻炼,更有利于提高患者临床疗效,改善其心肺功能及血气分析指标水平。
Objective:To analyze the efficacy of oxygen therapy combined with pulmonary rehabilitation exercise on chronic obstructive pulmonary disease(COPD)complicated with pulmonary heart disease.Methods:A total of 100 patients with COPD and pulmonary heart disease who were admitted to Qinghai Oilfield Hospital in Qili Town of Dunhuang City from June 2019 to June 2020 were selected as the study subjects,and they were randomly divided into the control group and the observation group,with 50 cases in each group.The control group received pulmonary rehabilitation exercise.The observation group received oxygen therapy on the basis of pulmonary rehabilitation exercise.The clinical efficacy,partial pressure of oxygen(PaO_(2)),partial pressure of carbon dioxide(PaCO_(2)),forced expiratory volume in 1 second(FEV_(1)),FEV_(1)/forced vital capacity(FVC)levels after 2 weeks of treatment,clinical symptom improvement time and improvement of St George's Respiratory Questionnaire(SGRQ),pulmonary artery systolic blood pressure,and cardiac Tei index were compared between the two groups.Results:The total effective rate of the observation group was higher than that of the control group.After 2 weeks of treatment,the levels of PaO_(2),FEV_(1),FEV_(1)/FVC were higher than those of the control group,the level of PaCO_(2)was lower than that of the control group,the improvement time of clinical symptoms was lower than that of the control group,and the SGRQ score,pulmonary artery systolic blood pressure,and cardiac Tei were lower than those of the control group.The above differences were statistically significant(P<0.05).Conclusion:The use of long-term oxygen therapy combined with pulmonary rehabilitation exercise for COPD patients with pulmonary heart disease is more conducive to improving the clinical efficacy of patients and improving their pulmonary function and blood gas analysis index levels.
