基于3D打印技术的经皮肾镜碎石术对肾结石患者肾损伤因子及神经体液因子的影响

摘要 目的探讨在肾结石患者治疗中应用基于3D打印技术的经皮肾镜碎石术对肾损伤因子及神经体液因子的影响。方法选取2019年1月至2020年1月在郑州颐和医院接受治疗的84例肾结石患者作为研究对象,根据随机数字表法将患者分为观察组(n=42)与对照组(n=42),对照组接受经皮肾镜碎石术治疗,观察组接受基于3D打印技术的经皮肾镜碎石术治疗,对两组围手术期指标、肾损伤因子、神经体液因子及并发症进行比较。结果观察组手术时间、拔管时间、住院时间明显短于对照组,差异有统计学意义(P<0.05);术中出血量、并发症发生率明显低于对照组,差异有统计学意义(P<0.05)。治疗后,两组肾损伤因子[胱抑素C(Cys-C)、肾损伤分子-1(KIM-1)、中性粒细胞明胶酶相关载脂蛋白(NGAL)]均明显提高,差异有统计学意义(P<0.05);神经体液因子[降钙素原(PCT)、超敏C-反应蛋白(hs-CRP)、血管紧张素Ⅱ(AngⅡ)]均明显下降,差异有统计学意义(P<0.05),且观察组肾损伤因子、神经体液因子水平明显低于对照组,差异有统计学意义(P<0.05)。结论基于3D打印技术的经皮肾镜碎石术应用于肾结石患者治疗中,能够改善围手术期指标,减少肾损伤,促进神经体液因子水平的下降,减少并发症发生。 Objective To investigate the effect of percutaneous nephrolithotomy based on 3D printing technology on renal injury factors and neurohumoral factors in the treatment of patients with renal calculi.Methods A total of 84 patients with renal calculi who were treated in Zhengzhou Yihe Hospital from January 2019 to January 2020 were selected as the research objects,and the patients were divided into the observation group(n=42)and the control group(n=42)according to the random number table method.The control group was received percutaneous nephrolithotomy,and the observation group was received percutaneous nephrolithotomy based on 3D printing technology.The perioperative indexes,renal injury factors,neurohumoral factors and complications were compared between the two groups.Results The operation time,extubation time and hospital stay in the observation group were significantly shorter than those in the control group(P<0.05);the intraoperative blood loss and the incidence of complications were significantly lower than those in the control group(P<0.05).After treatment,the renal injury factors[cystatin C(Cys-C),kidney injury molecule-1(KIM-1),and neutrophil gelatinase-associated apolipoprotein(NGAL)]in the two groups were significantly higher(P<0.05),neurohumoral factors[procalcitonin(PCT),high-sensitivity C-reactive protein(hs-CRP)and angiotensin II(AngⅡ)]were significantly lower(P<0.05),the levels of renal injury factors,neurohumoral factors in the observation group was significantly lower than those of the control group(P<0.05).Conclusion The application of percutaneous nephrolithotomy based on 3D printing technology in the treatment of patients with renal calculi can improve perioperative indicators,reduce renal injury,promote the decline of neurohumoral factor levels,and reduce the occurrence of complications.
