经阴道三维超声在黄素化未破裂卵泡综合征卵巢卵泡期血流灌注评价中的应用价值

Applied Value of Transvaginal Three-Dimensional Ultrasonography in Blood Perfusion Assessment for Follicular Phase of Luteinized Unruptured Follicle Syndrome

目的:探索在黄素化未破裂卵泡综合征(LUFS)卵巢卵泡期血流灌注评价中采取经阴道三维超声检测的临床价值。方法:选取2020年6月-2021年6月在中信湘雅生殖与遗传专科医院治疗的60例不孕症患者,其中30例LUFS患者为观察组,30例正常排卵女性为对照组,所有人员均在围排卵期给予经阴道三维超声检查,检查后对子宫内膜厚度进行评估,并对卵巢血流指数[血管化指数(VI)、血流指数(FI)、血管化血流指数(VFI)]以及功能侧卵巢动脉血流参数[搏动指数(PI)、阻力指数(RI)、收缩期峰值流速(S)、舒张末期流速(D)]进行检查。结果:对比对照组,观察组围排卵期S、D、VI、FI、VFI指标均较低,PI、RI指标均较高,差异有统计学意义(P<0.05);两组围排卵期子宫内膜厚度对比无明显差异(P>0.05);观察组与对照组围排卵期卵泡血流分级对比差异显著,差异有统计学意义(P<0.05)。结论:经阴道三维超声检查,可掌握LUFS的血流灌注特点,促使临床尽早识别、预测卵泡发育不良,进而为临床诊断、治疗提供一定的理论依据,值得借鉴。

Objective:To study the clinical value of transvaginal three-dimensional ultrasonography in the blood perfusion assessment for follicular phase of luteinized unruptured follicle syndrome(LUFS).Methods:Sixty infertility patients treated in Reproductive and Genetic Hospital of CITIC-Xiangya from June 2020 to June 2021 were selected.Among them,30 LUFS patients were taken as observation group and 30 women with normal ovulation were taken as control group.At peri-ovulatory phase,all subjects received the transvaginal three-dimensional ultrasonography.After examination,the endometrial thickness was evaluated,the ovarian blood flow indexes[vascularization index(VI),flow index(FI),vascularization flow index(VFI)]and ovarian arterial blood flow parameters at functioning side[pulsation index(PI),resistance index(RI),systolic blood flow velocity(S)and enddiastolic blood flow velocity(D)]were detected.Results:At peri-ovulatory phase,S,D,VI,FI and VFI values in observation group were lower than those in control group,while PI and RI values were higher than those in control group,the difference was statistically significant(P<0.05);at peri-ovulatory phase,there was no statistically significant difference in endometrial thickness between two groups(P>0.05);at peri-ovulatory phase,there was no statistically significant difference in follicular blood flow grading between two groups(P<0.05).Conclusion:The transvaginal three-dimensional ultrasonography can reflect the blood perfusion characteristics of LUFS,make the early clinical diagnosis and prediction of follicular dysplasia,and accordingly provide theoretical basis on the clinical diagnosis and treatment,and deserve a reference.
