2014—2019年上海口岸出入境人员HIV监测分析

Analysis of HIV surveillance among entry-exit personnel at Shanghai port from 2014 to 2019

目的评估上海口岸出入境人员HIV感染情况、流行趋势及危险因素。方法回顾性分析2014—2019年上海口岸出入境人员HIV监测结果,对HIV抗体阳性者进行流行病学调查,分析人群特征及危险因素。结果共调查出入境人员376165人,检出HIV抗体阳性263例,检出率为69.9/10万,其中87.5%为入境人员;检出率呈逐年上升趋势,2017年达高峰。HIV感染者以20~39岁男性为主,20岁以下青少年所占比例有所增加;HIV感染者以未婚、本科及以上文化程度、劳务人员为主。共进行263份有效流行病学调查,显示性传播仍是上海口岸HIV感染者的主要传播途径,尤其是男性同性性接触。共发现63例HIV/梅毒、3例HIV/乙肝、1例HIV/丙肝双重感染者。结论上海口岸出入境人员中HIV感染有所上升,口岸疾病监测应重点关注外籍入境劳务人员,同时应提倡国际旅行者的安全性行为。

Objective To evaluate the infection,prevalence and risk factors of HIV among entry-exit personnel at Shanghai port.Methods The surveillance data of HIV among entry-exit personnel at Shanghai port from 2014 to2019 was retrospectively analyzed,and epidemiological investigation was conducted on HIV antibody positive persons,the population characteristics and risk factors were analyzed.Results A total of 376165 people were investigated and 263 HIV antibody positive cases were detected.The detection rate of HIV was 69.9/100,000,of which 87.5%were entry persons.The detection rate showed an upward trend year by year,and reached the peak in 2017.Among the HIV infected people,most of them were aged 20-39 men,and the proportion of young people under 20 years old had increased.Most of the HIV infected people were unmarried,with bachelor degree or above and laborers.A total of 263 effective epidemiological questionnaires were carried out,the results showed that sexual transmission was still the main route of HIV infection at Shanghai port,especially homosexual contact between men.63 cases of HIV/syphilis,3 cases of HIV/hepatitis B and 1 case of HIV/hepatitis C co-infection were found.Conclusion HIV infection among the entry-exit personnel at Shanghai port had increased,so we should focus on the foreign labor workers in the port disease surveillance,at the same time,we should pay attention to the promotion of safe sexual behavior of international travelers.
