CRISPR技术用于新冠肺炎诊断的Meta分析

Meta-analysis of diagnostic value of CRISPR for COVID-19

摘要 目的 评价成簇规律间隔短回文重复序列(CRISPR)技术对新冠肺炎(COVID-19)的诊断价值。方法 在Pubmed、Embase、Cochrane Library、Wiley和Scopus以及中国知网、万方和维普数据等中外文数据库中以“新型冠状病毒肺炎”或“新型冠状病毒”“成簇规律间隔短回文重复序列”“敏感性和特异性”为主题词或关键词,检索CRISPR技术用于COVID-19诊断的相关文献,在筛选文献、质量评估以及数据提取后进行Meta分析。结果 共检索到354篇文献,最终纳入11篇文献15个研究,合并敏感性、特异性和诊断比值比分别为0.96(0.95,0.97),0.97(0.96,0.98)和682.97(393.62,1185.01),绘制综合受试者工作曲线,计算曲线下面积为0.9927,亚组分析和Meta回归分析均提示单靶、多靶基因检测的诊断准确性差异有统计学意义(P<0.05)。结论 CRISPR技术尤其是多靶标检测可用于诊断COVID-19,有较高的准确性,可用于大规模人群筛查。 Objective To analyze and evaluate the diagnostic value of clustered regularly interspaced short palindromic repeats(CRISPR) for COVID-19. Methods The articles about the diagnosis of COVID-19 by CRISPR,using COVID-19 or SARS-CoV-2,clustered regularly interspaced short palindromic repeats,sensitivity and specificity as MeSH or keywords,were retrieved from Pubmed,Embase,Cochrane Library,Wiley,Scopus,CNKI(China National Knowledge Infrastructure),Wanfang Database and VIP database. Meta-analysis was conducted after articles screening,quality assessment and data extraction. Results A total of 354 articles were retrieved and 11 articles including 15 studies were included ultimately. The pool sensitivity,specificity and diagnostic odds ratio(DOR) was 0.96(0.95,0.97),0.97(0.96,0.98) and 682.97(393.62,1185.01),respectively. The symmetric receiver operating characteristic curve(SROC) was made,and the area under curve(AUC) was 0.9927. Both subgroup analysis and Meta regression analysis showed that there was statistical difference in diagnostic accuracy between single and multiple target genes detection(P<0.05). Conclusion CRISPR technology,especially multi-targets testing,with high accuracy to diagnose COVID-19,can be used to screen for large population.
