高频彩色多普勒超声联合弹性成像在乳腺癌与乳腺增生结节中的鉴别诊断价值

Differential Diagnosis Value of High Frequency Color Doppler Ultrasound Combined with Elastography in Breast Cancer and Breast Hyperplasia

目的探讨高频彩色多普勒超声联合弹性成像在乳腺癌与乳腺增生结节鉴别诊断中的价值。方法选择2019年3月—2020年3月龙岩人民医院收治的以乳腺结节为主诉就诊的158例患者为研究对象,入组患者均为女性,统计入组患者穿刺活检或术后病理检查结果。以病理结果为金标准,分析高频彩色多普勒超声、弹性成像技术及联合检测对乳腺癌与乳腺增生结节诊断的灵敏度、特异度及准确度的差异。结果入组158例患者穿刺活检或术后病理检查结节病灶174个,以病理结果为金标准,174个结节病灶中良性病变116个(66.67%)、恶性病变58个(33.33%)。高频彩色多普勒超声对乳腺癌与乳腺增生结节诊断的灵敏度、特异度、准确度分别为70.69%、74.14%、72.99%;弹性成像技术为67.24%、71.55%、70.11%;联合检测为94.83%、93.10%、93.68%,联合检测的灵敏度、特异度、准确优于单一检测(P<0.05)。结论高频彩色多普勒超声联合弹性成像对乳腺癌与乳腺增生结节的诊断符合率较高,能有效避免单一检测技术的不足,值得临床推广。

Objective To explore the value of high frequency color Doppler ultrasound combined with elastography in differential diagnosis of breast cancer and breast hyperplasia nodules.Methods From March 2019to March 2020,a total of 158 patients with breast nodules as the chief complaint were selected as the research objects in Longyan People’s Hospital.All the patients in the study group were women.The results of puncture biopsy or postoperative pathological examination of the patients in the study group were statistically analyzed.Taking pathological results as gold standard,the differences in sensitivity,specificity and accuracy of high-frequency color Doppler ultrasound,elastography and combined detection in the diagnosis of breast cancer and breast hyperplasia nodules were analyzed.Results A total of 158 patients underwent needle biopsy or postoperative pathological examination of 174 nodular lesions.The pathological results were gold standard.There were 116 benign lesions(66.67%)and 58 malignant lesions(33.33%)in 174 nodular lesions.The sensitivity,specificity and accuracy of high-frequency color Doppler ultrasound in the diagnosis of breast cancer and breast hyperplasia nodules are 70.69%,74.14%and 72.99%,respectively.The elastic imaging technology is 67.24%,71.55%and 70.11%,and the combined detection is 94.83%,93.10%and 93.68%,respectively.The sensitivity,specificity and accuracy of the combined detection were significantly better than the single test(P<0.05).Conclusion Highfrequency color Doppler ultrasound combined with elastography has a high coincidence rate in the differential diagnosis of breast cancer and hyperplasia,and effectively avoids the deficiency of single detection technology,which is worthy of clinical promotion.
