生物反馈电刺激结合Kegel训练对产后盆底功能障碍患者康复的影响 被引量:6

摘要 目的探讨产后盆底功能障碍Kegel训练结合生物反馈电刺激对患者康复的影响。方法选取2020年4月至2021年5月景德镇市妇幼保健院收治的产后盆底功能障碍性疾病患者80例,随机分为对照组和治疗组,每组各40例。对照组采取生物反馈电刺激治疗,治疗组在对照组的基础上进行健康宣教,并指导患者进行正确Kegel训练。统计分析两组患者的盆底静息、收缩压力、盆底压力分级、阴道肌力、纤维疲劳指数、压力性尿失禁发生情况、盆底器官脱垂症状。结果治疗组患者治疗后较治疗前的盆底静息、收缩压力升高幅度均显著高于对照组(P<0.05)。治疗组患者的盆底压力分级Ⅰ级、Ⅱ级比例均高于对照组(P<0.05),Ⅲ级比例低于对照组(P<0.05)。治疗组患者的阴道肌力、Ⅰ级纤维疲劳指数均显著高于对照组(P<0.05),Ⅱ级纤维疲劳指数显著低于对照组(P<0.05)。治疗组患者治疗后较治疗前的压力性尿失禁发生率、盆底器官脱垂症状Ⅰ度、Ⅱ度比例降低幅度均显著高于对照组(P<0.05)。结论产后盆底功能障碍Kegel训练结合生物反馈电刺激较单独生物反馈电刺激更能有效促进患者康复。 Objective To investigate the effect of Kegel training combined with biofeedback and electrical stimulation on the rehabilitation of patients with postpartum pelvic floor dysfunction.Methods A total of 80 patients with postpartum pelvic floor dysfunction who were admitted to Jingdezhen Maternal and Child Health Hospital from April 2020 to May 2021 were selected and randomly divided into the control group and the treatment group,with 40 cases in each group.The control group received biofeedback and electrical stimulation,while the treatment group received health education on the basis of treatment of the control group,and patients in the two groups were instructed to correctly perform Kegel training.The pelvic floor resting and systolic pressures,pelvic floor pressure grading,occurrence of stress urinary incontinence(SUI),symptoms of pelvic organ prolapse(POP),urodynamic indexes,sexual function and clinical efficacy of patients in the two groups were statistically analyzed.Results The resting and systolic pressures of the pelvic floor in the treatment group after treatment were significantly higher than those in the control group(P<0.05).The proportions of pelvic floor pressure grades Ⅰ and Ⅱ in the treatment group were higher than those in the control group(P<0.05),and the proportion of gradeⅢwas lower than that in the control group(P<0.05).The incidence of SUI and the reductions in proportions of POP symptoms of grades Ⅰ and Ⅱ in the treatment group after treatment were significantly higher than those in the control group(P<0.05).After treatment,the maximum urinary flow rate,maximum bladder capacity,sexual frequency and orgasm frequency in the treatment group were higher than those in the control group(P<0.05).Conclusion Kegel training combined with biofeedback and electrical stimulation for postpartum pelvic floor dysfunction is more effective in promoting the recovery of patients than biofeedback and electrical stimulation alone.
