肛门环缩术联合消痔灵注射术治疗Ⅱ、Ⅲ度直肠脱垂的临床治疗效果

Clinical effect of anal constriction combined with Xiaozhiling injection in the treatment of gradeⅡandⅢrectal prolapse

目的研究肛门环缩术联合消痔灵注射术治疗Ⅱ、Ⅲ度直肠脱垂的临床治疗效果。方法选取2019年1月至2020年6月湖南中医药大学第二附属医院肛肠二科收治的Ⅱ、Ⅲ度直肠脱垂患者64例,随机分为观察组32例和对照组32例;观察组行肛门环缩术联合消痔灵黏膜下注射加直肠周围间隙注射术,对照组行消痔灵黏膜下注射加直肠周围间隙注射术。观察两组术后治疗总有效率、临床症状改善情况及并发症总发生率。结果术后半年观察组的治疗总有效率为100%,高于对照组的81.25%,差异有统计学意义(P<0.05);术后两组患者的肛门疼痛、肛门瘙痒潮湿、肛门坠胀均有改善,但差异无统计学意义(P>0.05);观察组的直肠脱出改善情况高于对照组,差异有统计学意义(P<0.05);观察组的并发症总发生率为9.37%,低于对照组的40.62%,差异有统计学意义(P<0.05)。结论肛门环缩术联合消痔灵注射术治疗Ⅱ、Ⅲ度直肠脱垂可有效提升患者的临床疗效,减少并发症,值得临床推广应用。

Objective To study the clinical therapeutic effect of anal constriction combined with Xiaozhiling injection in the treatment of gradeⅡandⅢrectal prolapse.Methods A total of 64 patients with gradeⅡandⅢrectal prolapse in the Second Department of Proctology of the Second Affiliated Hospital of Hunan University of Chinese Medicine from January 2019 to June 2020 were selected,and they were randomly divided into the observation group(n=32)and control group(n=32).The observation group received anal constriction combined with Xiaozhiling submucosal injection plus perirectal space injection,while the control group received Xiaozhiling submucosal injection plus perirectal space injection.The total effective rate,improvement of clinical symptoms and the total incidence of complications were observed.Results Six months after operation,the total effective rate of the observation group was 100%,which was higher than that(81.25%)of the control group,with statistically significant difference(P<0.05).The anal pain,anal itching and dampness and anal swelling were improved in both groups after operation,with no significant difference(P>0.05).The improvement of rectal prolapse in observation group was higher than that in control group,and the difference was statistically significant(P<0.05).The total incidence of complications in the observation group was 9.37%,which was lower than that(40.62%)in the control group,with statistically significant difference(P<0.05).Conclusion Anal constriction combined with Xiaozhiling injection can effectively improve the clinical efficacy and reduce complications of patients with gradeⅡandⅢrectal prolapse,which is worthy of clinical application.
