虎符铜砭循经刮痧对项痹病女性患者中医临床症状缓解效果的影响

Effect of Hufu Copper Bianstone scraping along the meridian on the relieving effect of TCM clinical symptoms in female patients with Xiangbi disease

目的:研究项痹病患者使用针灸联合虎符铜砭循经刮痧治疗后对中医症状的缓解效果。方法:选取2019年4月至2021年10月扬州市中医院中医经典科收治的80例项痹病女性患者作为研究对象,依据数字表法随机分为观察组和对照组,每组各40例;其中对照组行针灸治疗,并给予生活起居及功能锻炼指导,观察组在对照组基础上增加虎符铜砭循经刮痧(耳部全息及颈部经络)治疗;治疗周期2周,比较两组患者治疗前后的颈椎活动度和颈椎等长肌力;应用《中药新药临床研究指导原则》评价两组患者治疗前后的主要症状和体征分级量化积分;采用《颈椎病临床评价量表的信度与效度研究》评价两组患者治疗前后的生活、社会适应能力、心理和工作学习能力等;两组均随访2个月,观察复发情况。结果:观察组治疗后的近期疗效显示颈椎活动度、单个症状体征积分、总积分及生活、社会适应能力、心理和工作学习能力均较对照组有明显改善(P<0.05);治疗后,观察组患者的右旋和左旋颈椎活动度与对照组比较差异无统计学意义(P>0.05);随访2个月发现,观察组患者疾病复发率为7.5%(6/40),明显低于对照组的32.50%(13/40)(χ^(2)=15.374,P<0.001)。结论:针灸+虎符铜砭循经刮痧治疗项痹病可有效改善患者颈部功能、肌痉挛和体征,且此联合方法还可降低疾病复发率。

Objective:To study the relieving effect of TCM symptoms in patients with Xiangbi disease treated with acupuncture combined with Hufu Copper Bianstone and scraping along the meridian.Methods:Eighty female patients with arthralgia treated in the Department of Traditional Chinese Medicine Classics of Yangzhou Hospital of Traditional Chinese Medicine from April 2019 to October 2021 were selected as the research objects,and were randomly divided into observation group and control group according to the number table method,with 40 cases in each group.The control group received acupuncture and was given guidance on daily life and functional exercise.The observation group added Hufu Tongbian meridian scraping(ear hologram and neck meridian)therapy on the basis of the control group;the treatment period was 2 weeks,and the cervical spine range of motion and cervical isometric muscle strength of the two groups were compared before and after treatment;“Guiding Principles for Clinical Research of New Chinese Medicines”was applied to evaluate the main symptoms and signs of the two groups of patients before and after treatment;the“Reliability and validity of the Cervical Spondylopathy Clinical Evaluation Scale Study”was used to evaluate the life,social adaptability,psychological,work and learning abilities of the two groups of patients before and after treatment.Both groups were followed up for 2 months to observe the recurrence.Results:The short-term curative effect of the observation group after treatment showed that the range of motion of the cervical spine,the scores of individual symptoms and signs,the total score and life,social adaptability,psychology,work and study ability were significantly improved compared with those of the control group(P<0.05).There was no statistically significant difference between the right and left cervical vertebrae activity of the patients in the group and the control group(P>0.05);the 2-month follow-up found that the disease recurrence rate of the observation group was 7.5%(6/40),which was significantly lower than that of the control group(32.50%,13/40)(χ^(2)=15.374,P<0.001).Conclusion:Acupuncture combined with Hufu Copper Bianstone and scraping along the meridian can effectively improve the neck function,muscle spasm and physical signs of patients with Xiangbi disease,and this combined method can also reduce the recurrence rate of the disease.
