延续护理对老年糖尿病患者自我护理能力影响的Meta分析

Meta-analysis of the influence of continuous nursing on the self-care ability of elderly diabetic patients

目的通过Meta分析综合评价延续护理对老年糖尿病患者自我护理能力的干预效果。方法检索中国期刊网全文数据库(CNKI)、万方数据库、维普数据库、中国生物医学文献服务系统(Sinomed)、PubMed、Embase、Cochrane Library中关于老年糖尿病延续护理的随机对照研究,检索时间从建库到2021年5月,对纳入文献使用RevMan5.3软件进行Meta分析。结果共纳入文献14篇,共1667例患者,干预组834例,对照组833例;Meta分析结果显示,与常规护理相比,延续护理可提高老年糖尿病患者自我护理能力[SMD=1.39,95%CI(1.24,1.53),P<0.00001];自护责任感维度[SMD=1.18,95%CI(0.96,1.40),P<0.00001],自我概念维度[SMD=1.09,95%CI(0.97,1.21),P<0.00001],自我护理技能维度[SMD=1.25,95%CI(1.12,1.38),P<0.00001],健康知识维度[SMD=1.37,95%CI(1.04,1.70),P<0.00001],差异均具有统计学意义。结论延续护理对提高老年糖尿病患者自我护理能力具有积极作用,在延缓糖尿病进程,减少并发症发生,改善患者生活质量等方面具有重要意义。

Objective To comprehensively evaluate the effect of continuous nursing intervention on the self-care ability of elderly diabetic patients.Methods The Chinese Journals Full-text Database(CNKI),Wanfang Database,Weipu Database,Chinese Biomedical Document Service System(Sinomed),Pubmed,Embase,Cochrane Library and other databases were search for randomized controlled studies on continuation of elderly diabetes care.The retrieval time was from the establishment of the database to May 2021.Revman5.3 software was used to conduct meta-analysis.Results tA total of 14 articles were included in this study,with a total of 1667 patients,including 834 cases in the intervention group and 833 cases in the control group.Meta-analysis results showed that,compared with conventional care,continued care could improve the self-care ability of elderly diabetic patients[SMD=1.39,95%CI(1.24,1.53),P<0.00001].The Meta results of each dimension were as follow:self-care responsibility dimension[SMD=1.18,95%CI(0.96,1.40),P<0.00001];self-concept dimension[SMD=1.09,95%CI(0.97,1.21),P<0.00001];self-care skill dimension[SMD=1.25,95%CI(1.12,1.38),P<0.00001];health knowledge dimension[SMD=1.37,95%CI(1.04,1.70),P<0.00001],respectively,and all the differences had statistical significances.Conclusion Continued care has a positive effect on improving the self-care ability of elderly diabetic patients,and is of great significance in delaying the progress of diabetes,reducing complications,and improving the quality of life of patients.
