不同形式下经尿道手术对良性前列腺增生患者的临床效果

Clinical Effect of Different Forms of Transurethral Surgery on Patients With Benign Prostatic Hyperplasia

目的探究良性前列腺增生治疗中经尿道等离子前列腺电切术和经尿道1470 nm激光前列腺剜除术的临床效果。方法将84例在本院行手术治疗的前列腺增生患者作为对象,就诊时间为2020年1月—2021年2月,遵循数字标注法原则随机分组,A组(42例)行经尿道1470 nm激光前列腺剜除术治疗,B组(42例)行经尿道等离子前列腺电切术治疗,分析每组手术效果、术后生活质量以及术后尿失禁情况。结果与B组比较,A组患者术后住院时间、尿管留置时间短,手术出血量少,生活质量评分高,差异有统计学意义(P<0.05)。与B组比较,A组患者术后6个月IPSS评分低、Qmax高、RUV量少,差异有统计学意义(P<0.05)。与A组比较,B组患者术后短暂尿失禁发生率低,差异有统计学意义(P<0.05),但3个月后均恢复。结论良性前列腺增生患者在手术治疗中,经尿道1470 nm激光前列腺剜除术比经尿道等离子前列腺电切术在临床效果上有更大优势,更值得临床推广。

Objective To explore the clinical effect of transurethral plasmakinetic resection of prostate and transurethral 1470 nm laser enucleation of prostate in the treatment of benign prostatic hyperplasia.Methods A total of 84 patients with benign prostatic hyperplasia who underwent surgical treatment in our hospital were treated from January 2020 to February 2021.They were randomly divided into groups following the principle of number labeling.Group A(42 cases)underwent transurethral 1470nm laser enucleation of prostate For surgical treatment,group B(42cases)underwent transurethral plasma resection of the prostate.The surgical effect,postoperative quality of life and short-term postoperative urinary incontinence were analyzed in each group.Results Compared with group B,patients in group A had shorter postoperative hospital stay,shorter urinary catheter indwelling time,less operative blood loss,and higher quality of life scores(P<0.05).Compared with group B,patients in group A had lower IPSS score,higher Qmax,and lower RUV 6 months after operation,and the differences were statistically significant(P<0.05).Compared with group A,the incidence of postoperative transient urinary incontinence in group B was lower,and the difference was statistically significant(P<0.05),but all recovered after 3 months.Conclusion In the surgical treatment of patients with benign prostate hyperplasia,the urethra 1470 nm laser enucleation of the prostate has greater advantages in clinical effect than the urethra plasma electric prostate resection,which is more worthy of clinical promotion.
