康复期情境式交往训练对双相情感障碍患者的影响

Effect of Situational Communication Training During Rehabilitation on Patients With Bipolar Disorder

目的对康复期双相情感障碍患者实施情境式交往训练,评价其对自我效能和社会功能的改善作用。方法选取汕头市第四人民医院2018年1月至2019年12月期间收治的160例双相情感障碍患者经急性期治疗后进入康复期,在知情同意下,按住院号先后顺序分为研究组(完成77例)和对照组(完成74例),两组在药物维持治疗和常规康复训练的基础上,研究组实施8周情境式交往训练,对照组实施8周常规康复活动,两组患者实施干预前后评定汉密顿抑郁量表(Hamiiton depression scale,HAMD)、Bech-Rafaelsen躁狂量表(Bech-Rafaelsen mania scale,BRMS-R)、自我效能感量表(general self-efficacy scale,GSES)、社会功能缺陷筛选量表(social disability screening scale,SDSS)评分情况及两组患者干预效果,对结果进行分析评价。结果在实施干预后的8周内,研究组患者的HAMD、BRMS-R及SDSS等量表评分均明显低于对照组(P<0.01);两组的GSES评分和干预总有效率同样为研究组更高(P<0.05)。结论情境式交往训练对康复期双相情感障碍患者的自我效能感和社会功能有改善作用,值得临床推广应用。

Objective To evaluate the effects of situational communication training on the self-efficacy and social funtions in patients with bipolar disorder during rehabilitation.Methods A total of 160 patients with bipolar disorder were selected and admitted to the Fourth People′s Hospital of Shantou from January 2018 to December 2019,entering the rehabilitation period after acute treatment,which were randomly divided into the study group(completed 77 cases)and the control group(completed 74 cases)according to their hospitalization number and with informed consent.On the basis of drug maintenance treatment and routine rehabilitation training,the study group implemented 8 weeks of situational communication training,the control group carried out 8 weeks of routine rehabilitation activities.Before and after the intervention,two groups of patients were assessed the Hamilton depression scale(HAMD),the Bech-Rafaelsen mania scale(BRMS-R),the general self-efficacy scale(GSES),the social disability screening scale(SDSS)and the intervention effects of two groups of patients.The results and the scores were analyzed and evaluated.Results Within 8 weeks after the intervention,the HAMD,BRMS-R and SDSS scores of the patients in the study group were significantly lower than those in the control group(P<0.01);the GSES score and the total effective rate of intervention in the two groups were also higher than those in the study group(P<0.05).Conclusion Situational communication training has improved self-efficacy and social function in patients with bipolar disorder during rehabilitation.It is worthy of clinical promotion.
