糖化血红蛋白联合血脂指标检测对糖尿病的预测价值研究

Study on the Predictive Value of Glycosylated Hemoglobin Combined with Blood Lipid Index Detection in Diabetes Mellitus

目的通过研究糖化血红蛋白、血脂指标,分析其对糖尿病的预测价值,为糖尿病的预防提供依据。方法采用便利抽样法,选取该院确诊为2型糖尿病的患者500例作为观察组,并选取同一时期内500名健康志愿者为对照组。对比两组研究对象一般情况比较,并作出糖尿病的多因素Logistic回归分析,判断糖化血红蛋白、血脂指标是否为糖尿病的独立危险因素。用糖化血红蛋白、血脂指标对糖尿病进行ROC曲线分析,评估糖化血红蛋白、血脂指标对糖尿病的预测价值。结果空腹血糖、糖化血红蛋白对糖尿病的发生具有较好的预测价值,空腹血糖、糖化血红蛋白ROC曲线下面积为:0.739(95%CI:0.622~0.796)、0.867(95%CI:0.808~0.925)。TG、TC对糖尿病的发生具有较好的预测价值,ROC曲线下面积分别为:0.692(95%CI:0.622~0.762)、0.737(95%CI:0.668~0.805)。结论血脂、糖化血红蛋白对糖尿病具有较好的预测价值,值得推广。

Objective To analyze the predictive value of diabetes mellitus by studying glycosylated hemoglobin and blood lipid indexes,so as to provide the basis for the prevention of diabetes mellitus.Methods Using convenience sampling method,500 patients diagnosed with type 2 diabetes mellitus in the hospital were selected as observation group.And 500 healthy volunteers in the same period were selected as the control group.The general conditions of the two groups of subjects were compared,and the multivariate logistic regression analysis of diabetes was made.To determine whether glycosylated hemoglobin and blood lipid indexes are independent risk factors for diabetes.ROC curve analysis of diabetes was carried out with glycosylated hemoglobin and blood lipid indexes.To evaluate the predictive value of glycosylated hemoglobin and blood lipid indexes in diabetes.Results Fasting blood glucose and glycated hemoglobin had good predictive value for the occurrence of diabetes.The areas under the ROC curve of fasting blood glucose and glycated hemoglobin were 0.739(95%CI:0.622-0.796)and 0.867(95%CI:0.808-0.925).TG and TC have good predictive value for the occurrence of diabetes,and the areas under the ROC curve were 0.692(95%CI:0.622-0.762)and 0.737(95%CI:0.668-0.805),respectively.Conclusion Blood lipids and glycosylated hemoglobin have good predictive value for diabetes and are worthy of promotion.
