个案管理护理模式对糖尿病患者糖化血红蛋白控制率及生活质量的影响研究

Influence of Case Management Nursing Mode on Glycated Hemoglobin Control Rate and Quality of Life in Diabetic Patients

目的分析个案管理护理模式对糖尿病患者糖化血红蛋白控制率及生活质量的影响。方法选取2020年2月—2021年6月该院收治的糖尿病患者110例,经随机数表法将其分为两组,将其中接受常规护理的55例患者设为对照组,将另外接受个案管理护理模式的55例患者设为研究组,观察比较两组的糖化血红蛋白控制率及生活质量。结果相比于对照组,研究组护理后的糖化血红蛋白控制良好率明显提高,差异有统计学意义(P<0.05)。相比于护理前,两组护理后的空腹血糖、餐后2 h血糖水平均明显降低,差异有统计学意义(P<0.05);而相比于对照组,研究组护理后的空腹血糖、餐后2 h血糖水平降低幅度更加明显,差异有统计学意义(P<0.05)。相比于护理前,两组护理后的各项生活质量评分均显著提高,差异有统计学意义(P<0.05);而相比于对照组,研究组护理后的各项生活质量评分提高幅度更加明显,差异有统计学意义(P<0.05)。结论为糖尿病患者提供个案管理护理模式,能让糖化血红蛋白控制率明显提高,降低血糖水平,并让其生活质量显著提升,值得临床推广。

Objective To analyze the effect of case management nursing mode on the control rate of glycated hemoglobin and quality of life in diabetic patients.Methods A total of 110 diabetic patients admitted to the hospital from February 2020 to June 2021 were selected.They were divided into two groups by random number table method,and 55 patients who received routine care were set as the control group.An additional 55 patients who received the case management care model were included in the study group.The glycated hemoglobin control rate and quality of life of the two groups were observed and compared.Results Compared with the control group,the good control rate of glycosylated hemoglobin in the study group after nursing was significantly improved,and the difference was statistically significant(P<0.05).Compared with before nursing,the fasting blood glucose and 2 h postprandial blood glucose levels of the two groups after nursing were significantly decreased,and the difference was statistically significant(P<0.05).Compared with the control group,the fasting blood glucose and 2 h postprandial blood glucose levels of the study group decreased more significantly after nursing,and the difference was statistically significant(P<0.05).Compared with before nursing,the quality of life scores of the two groups after nursing were significantly improved,and the difference was statistically significant(P<0.05).Compared with the control group,the improvement of various quality of life scores in the study group after nursing was more obvious,and the difference was statistically significant(P<0.05).Conclusion Providing a case management nursing model for diabetic patients can significantly improve the glycated hemoglobin control rate,reduce blood glucose levels,and significantly improve their quality of life,which is worthy of clinical promotion.
