MRI动态增强扫描联合DWI序列对女性乳腺疾病性质鉴别诊断价值研究 被引量:19

摘要 目的探究MRI动态增强扫描联合DWI序列对女性乳腺疾病性质鉴别诊断价值。方法回顾性分析本院2017年6月至2019年8月收治的72例乳腺疾病患者的临床资料,将MRI动态增强扫描联合DWI检查的诊断结果进行讨论和分析;以病理检查结果为“金标准”,计算经MRI动态增强扫描联合DWI检查对乳腺疾病的诊断准确性、敏感性和特异性。并比较良、恶性乳腺疾病的形态及强化特征。结果72例乳腺疾病患者,经MRI动态增强扫描联合DWI检查出恶性疾病患者22例,良性疾病患者44例,诊断准确率为91.67%,敏感性为95.65%,特异性为89.80%;恶性乳腺疾病形态规则、边界不模糊、强化均匀所占的比例均低于良性乳腺疾病,两者比较差异有统计学意义(P<0.05)。结论MRI检查可有效显示乳腺疾病的影像学特点,动态增强扫描联合DWI在鉴别诊断良、恶性乳腺疾病具有重要的价值,可为临床治疗乳腺疾病提供可靠的理论依据,值得广泛推广应用。 Objective To explore value of MRI dynamic enhanced scanning combined with DWI sequence in the differential diagnosis of the nature of female breast disease.Methods the clinical data of 72 patients with breast diseases admitted to our hospital from June 2017 to August 2019 were retrospectively analyzed.the diagnosis results of MRI dynamic enhanced scan combined with DWI were discussed and analyzed.the results of pathological examination were“gold standard”to calculate the diagnostic accuracy,sensitivity and specificity of MRI dynamic enhanced scan combined with DWI for breast cancer disease.the shape and strengthening characteristics of benign and malignant breast diseases were compared.Results In 72 patients with breast disease,22 patients were diagnosed with malignant disease and 44 patients were diagnosed with benign disease by MRI dynamic enhanced scan combined with DWI.the diagnostic accuracy was 91.67%,the sensitivity was 95.65%,and the specificity was 89.80%.And the proportion of malignant breast disease with regular shape,non-blurred boundary and uniform enhancement was lower than those of benign breast disease,and the difference was statistically significant(P<0.05).Conclusion MRI examination can effectively display the imaging features of breast diseases.Dynamic enhanced scanning combined with DWI is of great value in the differential diagnosis of benign and malignant breast diseases.It can provide a reliable theoretical basis for clinical treatment of breast diseases.It is worthy of widespread application.
