针灸刺激联合吞咽功能训练对脑卒中吞咽障碍患者的干预效果

Intervention effect of acupuncture stimulation combined with swallowing training on stroke patients with dysphagia

目的探讨针灸刺激联合吞咽功能训练对脑卒中吞咽障碍患者生活质量和并发症的影响。方法以临沭县人民医院2018年6月至2020年6月收治的104例脑卒中吞咽障碍患者为研究对象,按随机数字表法分为对照组(52例)和观察组(52例)。对照组患者接受吞咽功能训练,观察组患者在吞咽功能训练的基础上加用针灸刺激。对比2组患者的吞咽功能、胃管拔除时间、生活质量和并发症。结果干预后1个月和3个月,2组的Gugging吞咽功能评估表(GUSS)和吞咽障碍程度评分均较同组干预前升高(P均<0.05),且观察组的上述评分均明显高于同期对照组(P均<0.05)。干预后1个月和3个月,2组的吞咽障碍特异性生活量表(SWAL-QOL)评分均较同组干预前升高(P均<0.05),且观察组的SWAL-QOL评分均高于同期对照组(P均<0.05)。观察组的胃管拔除时间早于对照组(P<0.05),并发症总发生率低于对照组(P<0.05)。结论吞咽功能训练联合针灸刺激可有效改善脑卒中吞咽障碍患者的吞咽功能,缩短留置胃管时间,并减少吞咽障碍相关并发症,提高生活质量。

Objective To explore the effect of acupuncture stimulation combined with swallowing training on the quality of life and complications of stroke patients with dysphagia.Methods A total of 104 stroke patients with dysphagia who were admitted to Linshu County People's Hospital from June 2018 to June 2020 were selected as the research objects.They were divided into a control group(52 cases)and an observation group(52 cases)according to the random number table method.The patients in the control group received swallowing training,and the patients in the observation group received acupuncture stimulation on the basis of swallowing training.The swallowing function,time of gastric tube removal,quality of life,and complications were compared between the two groups.Results After 1 month and 3 months of intervention,the scores of Gugging swallowing screen(GUSS)and swallowing dysfunction degree in the two groups were higher than those before the intervention(all P<0.05),and the above scores of the observation group were significantly higher those of the control group(all P<0.05).After 1 month and 3 months of intervention,the swallowing quality of life questionnaire(SWAL-QOL)scores of the two groups were higher than those before the intervention(all P<0.05),and the SWAL-QOL scores of the observation group were higher than those of the control group(both P<0.05).The time of gastric tube removal of the observation group was earlier than that of the control group(P<0.05),and the total incidence of complications was lower than that of the control group(P<0.05).Conclusion Swallowing training combined with acupuncture stimulation can effectively improve the swallowing function of stroke patients with dysphagia,shorten the gastric tube indwelling time,reduce the complications related to dysphagia,and improve the quality of life.
