^(18)F-FDG PET/CT诊断肝细胞癌及胆管细胞癌的准确性比较

A comparison between the diagnostic accuracies of ^(18)F-FDG PET/CT for hepatocellular carcinoma and cholangiocarcinoma

摘要 目的比较18氟-氟代脱氧葡萄糖(^(18)F-FDG)正电子发射断层显像术/X线计算机体层摄影术(PET/CT)诊断肝细胞癌(HCC)及胆管细胞癌(CCC)的准确性。方法收集132例肝癌患者的^(18)F-FDG PET/CT影像学资料,其中HCC患者90例,CCC患者42例。将病灶^(18)F-FDG浓度高于周围正常肝组织诊断为阳性。通过一致性分析,评价^(18)F-FDG PET/CT对HCC及CCC的诊断价值。结果HCC患者SUVmax值(5.12±2.03)显著低于CCC患者(7.64±2.14),差异有统计学意义(t=6.529,P=0.000)。经一致性分析,90例HCC患者中^(18)F-FDG PET/CT阳性52例,灵敏度为0.577,特异度为0.786,准确率为0.644,Kappa为0.307。经一致性分析,42例CCC患者中^(18)F-FDG PET/CT阳性39例,灵敏度为0.929,特异度为0.978,准确率为0.962,Kappa为0.912。结论^(18)F-FDG PET/CT诊断CCC的SUVmax值显著高于HCC,准确率也显著高于HCC。 Objective To compare the accuracies of ^(18)F-FDG positron emission tomography/X-ray computed tomography(PET/CT)for the diagnosis of hepatocellular carcinoma(HCC)and cholangiocarcinoma(CCC).Methods The ^(18)F-FDG PET/CT imaging data of 132 patients with liver cancer were collected.There were 90 patients with HCC and 42 patients with CCC.^(18)F-FDG concentration higher than the surrounding normal liver tissue was diagnosed as positive.The values of ^(18)F-FDG PET/CT in the diagnosis of HCC and CCC were evaluated by consistency analysis.Results The SUVmax value of HCC patients was 5.12±2.03,which was significantly lower than that of 7.64±2.14 in CCC patients(t=6.529,P=0.000).By consistency analysis,^(18)F-FDG was found positive in 50 of the 90 HCC patients,with sensitivity,specificity,accuracy and kappa values of 0.577,0.786,0.644 and 0.307,respectively.The consistency analysis of ^(18)F-FDG in CCC patients shown that 39 of the 42 CCC patients were positive,with sensitivity,specificity,accuracy and kappa values of 0.929,0.978,0.962 and 0.912,respectively,which were higher than those of HCC patients.Conclusion The SUVmax value of ^(18)F-FDG PET/CT for diagnosing CCC was significantly higher than that of HCC,so as to the accuracy.
