推拿干预腰椎间盘突出症相关疼痛的机制探讨

Discussion on analgesic mechanism of Tuina intervention on lumbar disc herniation

腰椎间盘突出症(LDH)作为临床常见退行性脊柱病之一,以腰痛和下肢放射痛为主要症状。作为干预LDH的主要保守疗法之一,推拿能通过对肌肉、组织、关节等产生的机械作用,从而缓解疼痛。然而,关于其镇痛机制研究还相对不足,缺乏一定的理论依据。随着LDH相关疼痛的多重机制逐渐被认识,推拿镇痛的多靶点效应也应当在此基础上加以阐释。因此,文章总结探讨了近年来推拿干预LDH的相关研究,从而为推拿的镇痛机制提供了一定的理论依据,以期从中医理论、神经生物学、生物力学等学科交叉中探索出一整套科学的推拿干预体系,从而有效防治LDH。

Lumbar disc herniation(LDH)is one of the most common degenerative spinal diseases with low back pain and radiation pain of lower limbs.As one of the main conservative therapies for LDH,Tuina can relieve pain through the mechanical effects on muscles,tissues and joints.However,the research on the analgesic mechanism of Tuina is insufficient and lacks theoretical basis.Since more study recognized the multiple mechanisms of the pain is related to LDH,the multi-target analgesia mechanisms of Tuina should also be discussed.Therefore,this paper summarized and discussed the researches of Tuina on LDH in recent years,to provide some theoretical basis for the analgesic mechanism of Tuina.We also aimed to explore a complete set of scientific Tuina intervention system from the interdisciplinary intersection of traditional Chinese medicine,neurobiology,biomechanics and so on.In this way,we present Tuina as a new method to effectively prevent and treat LDH.
