精细化手术室护理对行全膝关节置换术患者的影响

Effect of refined nursing in operating room on patients undergoing total knee arthroplasty

目的探讨行全膝关节置换术患者接受精细化手术室护理,对其术后并发症发生情况、疼痛程度和膝关节功能的影响。方法选择2020年6月至2021年1月在徐州医科大学附属医院接受全膝关节置换术的102例患者为研究对象,采用方便抽样法分为对照组(51例)和观察组(51例)。对照组患者采用常规护理,观察组患者基于常规护理采用精细化手术室护理。比较2组患者的疼痛程度、膝关节功能及术后感染等并发症发生情况。结果术前,2组间视觉模拟评分法(VAS)评分的差异无统计学意义(P>0.05)。术后12 h、术后24 h和术后3 d,2组的VAS评分均低于同组术前(P均<0.05),且观察组均低于对照组(P均<0.05)。术后1个月,观察组的疼痛、关节功能、屈曲畸形、关节稳定性和肌力水平评分均高于对照组(P均<0.05)。观察组的并发症总发生率低于对照组(P<0.05)。观察组对护理人员护理服务态度、操作技能、护理全面性和护理安全性的评分均高于对照组(P均<0.05)。结论患者在行全膝关节置换术治疗期间实施精细化手术室护理,有助于缓解其疼痛程度,在促进其膝关节功能恢复的同时对术后并发症具有理想的预防效果,且能够提高护理满意度。

Objective To explore the effect of refined nursing in operating room on the postoperative complications,pain intensity,and knee function of patients undergoing total knee arthroplasty.Methods A total of 102 patients who underwent total knee arthroplasty at the Affiliated Hospital of Xuzhou Medical University from June 2020 to January 2021 were selected.The participants were divided into the control group(51 cases)and the observation group(51 cases)by convenience sampling method.The control group adopted routine nursing,and the observation group adopted refined operating room nursing based on the routine nursing.The pain intensity,knee function,postoperative infection and other complications were compared between the two groups.Results Before the surgery,there was no significant difference in visual analogue scale(VAS)scores between the two groups(P>0.05).At 12 hr,24 hr and 3 days after surgery,the VAS scores of the two groups were lower than those before the surgery(all P<0.05),and the scores of the observation group were lower than those of the control group(all P<0.05).One month after surgery,the scores of pain,joint function,flexion deformity,joint stability,and muscle strength in the observation group were higher than those in the control group(all P<0.05).The incidence of complications in the observation group was lower than that in the control group(P<0.05).The scores of nursing service attitude,operation skills,nursing comprehensiveness,and nursing safety in the observation group were higher than those in the control group(all P<0.05).Conclusion The application of refined nursing in the operating room can relieve the pain of patients undergoing total knee arthroplasty,promote the recovery of knee function,and have an ideal preventive effect on postoperative complications.It can also improve the nursing satisfaction.
