D-D、CRP、LDH在重症支原体肺炎患儿中的变化及对病情的诊断价值 被引量:3

Changes of D-D, CRP, and LDH in Children with Severe Mycoplasma Pneumonia and Their Diagnostic Values

摘要 目的分析D-二聚体(D-D)、C反应蛋白(CRP)、乳酸脱氢酶(LDH)在重症支原体肺炎(MPP)患儿中的变化及对病情的诊断价值。方法选取2016年6月—2021年6月收治的124例MPP(MPP组),根据病情不同分为重症组58例与轻症组66例。另选取同期收治的细菌性肺炎(细菌组)102例及病毒性肺炎(病毒组)97例。对比不同人群D-D、CRP、LDH水平,受试者工作特征曲线分析D-D、CRP、LDH对重症MPP的诊断价值。结果MPP组D-D及LDH水平均高于病毒组和细菌组,而CRP水平则低于细菌组、高于病毒组(P<0.05)。重症组D-D、CRP、LDH水平均高于轻症组(P<0.05),重症MPP患儿急性期D-D、CRP、LDH水平均高于恢复期(P<0.05)。D-D、CRP、LDH联合检测诊断重症MPP的曲线下面积为0.817。结论D-D、CRP、LDH水平在重症MPP患儿中明显上升,且与病情严重程度存在一定的联系,三者联合检测可提高对重症MPP的诊断效能。 Objective To analyze the changes of D-dimer(D-D),C-reactive protein(CRP)and lactate dehydrogenase(LDH)in children with severe mycoplasma pneumonia(MPP)and their diagnostic values.Methods A total of 124 MPP cases(MPP group)who were admitted from June 2016 to June 2021 were selected and divided into severe case group(n=58)and mild case group(n=66)according to different conditions.In addition,102 cases of bacterial pneumonia(bacterial group)and 97 cases of viral pneumonia(viral group)were selected for treatment during the same period.The levels of D-D,CRP,and LDH in different populations were compared,and receiver operating characteristic(ROC)curve was drawn to analyze the diagnostic values of D-D,CRP,and LDH for severe MPP.Results The levels of D-D and LDH in the MPP group were higher than those in the virus group and the bacteria group,while the CRP level was lower than that in the bacteria group and higher than that in the virus group(P<0.05).The levels of D-D,CRP and LDH in the severe group were higher than those in the mild group(P<0.05),and the levels of D-D,CRP and LDH in the acute stage of children with severe MPP were higher than those in the recovery stage(P<0.05).The area under the ROC curve of the combined detection of D-D,CRP and LDH for the diagnosis of severe MPP was 0.817.Conclusion The levels of D-D,CRP,and LDH are significantly increased in children with severe MPP,which have a certain relationship with the severity of the disease.The combined detection of the three can improve the diagnostic efficiency of severe MPP.
