根面平整联合治疗中重度慢性牙周炎在2型糖尿病患者中的应用价值

Application of subgingival scaling in treatment of moderate and severe chronic periodontitis in patients with type 2 diabetes mellitus

目的 观察龈下刮治根面平整联合治疗2型糖尿病中重度慢性牙周炎患者中的应用价值。方法 选取2019年8月-2021年8月在聊城市中医医院72例2型糖尿病合并中重度慢性牙周炎患者,分为联合组、根面平整组、龈下刮治组、清洁组各18例,比较四组患者治疗前及治疗3个月后血糖、菌斑指数(PI)、牙龈指数(GI)、探诊深度(PD)、附着丧失(AL)、炎症因子α(TNF-α)和超敏C-反应蛋白(hs-CRP)及生活质量差异。结果 四组治疗前各指标差异均无统计学意义(P>0.05),治疗3个月后,联合组PI、GI、PD、AL、空腹血糖、餐后2 h血糖、糖化血红蛋白、TNF-α、hs-CRP均低于其他三组,差异有统计学意义(P<0.05),联合组治疗维度、社会关系、心理状况和生理功能评分均高于其他三组,差异有统计学意义(P<0.05)。结论 龈下刮治根面平整联合治疗对2型糖尿病合并中重度慢性牙周炎有良好的治疗作用,对维持血糖水平、提高患者生活质量具有重要作用。

Objective To observe the application value of subgingival curettage and root planing in the treatment of moderate to severe chronic periodontitis in type 2 diabetes.Methods A total of 72 patients with type 2 diabetes mellitus and moderate to severe chronic periodontitis diagnosed and treated from Aug. 2019 to Aug. 2021 were divided into combined group, root planing group, subgingival curettage group, and cleaning group with 18 cases each.The blood glucose, plaque index(PI),gingival index(GI),probing depth(PD),attachment loss(AL),TNF-α,hs-CRP and differences in quality of life of 4 groups were compared before treatment and after 3 months of treatment.Results There was no statistical difference in the indicators of the 4 groups before treatment(P>0.05).After 3 months of treatment, the combined group had PI,GI,PD,AL,fasting blood glucose, 2 h postprandial blood glucose, glycosylated hemoglobin, TNF-α,hs-CRP was lower than the other 3 groups, and the difference was statistically significant(P<0.05).The scores of treatment dimension, social relationship, psychological status and physiological function in the combined group were all higher than the other three groups, and the difference was statistically significant(P<0.05).Conclusion The combined treatment of subgingival curettage and root planing has a good therapeutic effect on type 2 diabetes with moderate to severe chronic periodontitis, and has an important effect on maintaining blood sugar levels and improving the quality of life of patients.
