经尿道等离子腔内前列腺剜除术对良性前列腺增生患者尿动力学指标、COX-2、VEGF水平的影响

Effects of transurethral plasma cavity prostatic enucleation on urodynamic indexes,COX-2 and VEGF levels in patients with benign prostatic hyperplasia

目的探讨经尿道等离子腔内前列腺剜除术治疗良性前列腺增生及对患者尿动力学指标、环氧合酶-2(COX-2)、血管内皮生长因子(VEGF)水平的影响。方法选取2017年1月至2020年12月我院收治的60例良性前列腺增生患者,采用随机数字表法将其分为对照组和观察组,各30例。对照组采用经尿道前列腺电切术治疗,观察组采用经尿道等离子腔内前列腺剜除术治疗。比较两组患者的尿动力学指标、并发症发生情况、炎性因子水平、应激指标及COX-2、VEGF水平。结果术后1个月,两组患者的Qmax、BC均升高,PVR均降低,且观察组优于对照组(P<0.05)。观察组的术后并发症总发生率低于对照组(P<0.05)。术后1个月,两组患者的CRP、E及Cor水平均升高,但观察组低于对照组(P<0.05)。术后1个月,两组患者的COX-2、VEGF水平均降低,且观察组低于对照组(P<0.05)。结论经尿道等离子腔内前列腺剜除术治疗良性前列腺增生有助于改善患者的尿动力学指标,减少术后并发症,减轻炎症、应激反应,降低COX-2、VEGF水平,值得推广。

Objective To investigate the effects of transurethral plasma cavity prostatic enucleation on urodynamic indexes,cyclooxygenase-2(COX-2)and vascular endothelial growth factor(VEGF)levels in patients with benign prostatic hyperplasia.Methods A total of 60 patients with benign prostatic hyperplasia treated in our hospital from January 2017 to December 2020 were selected,and the patients were divided into control group and observation group according to random number table method,with 30 cases in each group.The control group was treated with transurethral resection of the prostate,and the observation group was treated with transurethral plasma cavity prostatic enucleation.The urodynamic indexes,complications,inflammatory factors levels,stress indexes,COX-2 and VEGF levels were compared between the two groups.Results One month after operation,Qmax and BC in both groups increased,PVR decreased,and those in the observation group were better than the control group(P<0.05).The total incidence of postoperative complications in the observation group was lower than that in the control group(P<0.05).One month after operation,the levels of CRP,E and Cor in the two groups increased,but those in the observation group were lower than the control group(P<0.05).One month after operation,the levels of COX-2 and VEGF in the two groups decreased,and those in the observation group were lower than the control group(P<0.05).Conclusion Transurethral plasma cavity prostatic enucleation in the treatment of benign prostatic hyperplasia is helpful to improve the urodynamic indexes,decrease postoperative complications,inflammation and stress responseand,reduce the levels of COX-2 and VEGF of patients,which is worthy of promotion.
