强脉冲光联合睑板腺按摩治疗睑板腺功能障碍型白内障术后干眼症的临床效果

Clinical effect of intense pulsed light combined with meibomian gland massage in the treatment of dry eye after surgery for cataract with meibomian gland dysfunction

目的探究强脉冲光联合睑板腺按摩治疗睑板腺功能障碍型白内障术后干眼症的临床效果。方法选择我院2020年7月至2021年7月收治的70例睑板腺功能障碍型白内障术后干眼症患者,按随机数字表法将其分为对照组和观察组,每组35例。对照组采取睑板腺按摩治疗,观察组采取强脉冲光联合睑板腺按摩治疗。比较两组的感觉性症状、眼表体征、眼表疾病指数(OSDI)评分、泪膜破裂时间(BUT)、泪河高度、睑板开口状态评分及临床疗效。结果治疗后,两组的感觉性症状、眼表体征评分均较治疗前显著降低,且观察组低于对照组,差异具有统计学意义(P<0.05)。治疗后,两组的OSDI评分显著低于治疗前,BUT显著长于治疗前,且观察组优于对照组,差异具有统计学意义(P<0.05)。治疗后,两组的泪河高度大于治疗前,睑板开口状态评分低于治疗前,且观察组优于对照组,差异具有统计学意义(P<0.05)。观察组的治疗总有效率显著高于对照组,差异具有统计学意义(P<0.05)。结论强脉冲光联合睑板腺按摩治疗睑板腺功能障碍型白内障术后干眼症的效果显著,可明显改善患者的临床症状及体征,降低OSDI及睑板开口状态评分,延长BUT,增大泪河高度。

Objective To investigate the clinical effect of intense pulsed light combined with meibomian gland massage in the treatment of dry eye after surgery for cataract with meibomian gland dysfunction.Methods Seventy patients with dry eye after surgery for cataract with meibomian gland dysfunction treated in our hospital from July 2020 to July 2021 were selected.According to random number table method,the patients were divided into control group and observation group,with 35 cases in each group.The control group was treated with meibomian gland massage,and the observation group was treated with intense pulsed light combined with meibomian gland massage.The sensory symptom,ocular surface sign,ocular surface disease index(OSDI)scores,tear film break-up time(BUT),tear meniscus height,tarsal opening status score and clinical efficacy were compared between the two groups.Results After treatment,the scores of sensory symptom and ocular surface sign in the two groups were significantly lower than those before treatment,those in the observation group were lower than the control group,and the differences were statistically significant(P<0.05).After treatment,OSDI score of the two groups was significantly lower than that before treatment,BUT was significantly longer than that before treatment,those in the observation group were better than the control group,and the differences were statistically significant(P<0.05).After treatment,the tear meniscus height in the two groups was greater than that before treatment,the tarsal opening state score was lower than that before treatment,those in the observation group were better than the control group,and the differences were statistically significant(P<0.05).The total effective rate of treatment in the observation group was significantly higher than that in the control group,and the difference was statistically significant(P<0.05).Conclusion Intense pulsed light combined with meibomian gland massage in the treatment of dry eye after surgery for cataract with meibomian gland dysfunction has a significant effect.It can significantly improve the clinical symptoms and signs of patients,reduce the OSDI and tarsal opening status scores,prolong BUT and increase the tear meniscus height.
