细节优质护理对腹腔镜阑尾切除术患者的影响

Effect of Quality Care in Detail on Patients Undergoing Laparoscopic Appendectomy

目的探索细节优质护理在腹腔镜阑尾切除术患者中的应用效果。方法于本院普外科2018年5月至2019年10月收诊的行腹腔镜阑尾切除术患者中选出210例为对象,以随机数字表法分组:对照组患者给予常规护理措施,观察组患者在此基础上给予细节优质护理,收集两组患者的术后并发症发生率、术后住院时间、护理满意度、术前术后心理状态评分等。结果观察组患者的术后并发症发生率(3.81%)低于对照组(11.43%),P<0.05;且观察组护理满意度97.14%高于对照组的护理满意度88.57%,P<0.05;观察组患者术前2 h、术后24 h的SAS、SDS评分以及术后首次下床活动时间、首次排气时间、首次肠鸣音恢复时间、首次排便时间、住院时间均明显优于对照组,P<0.001;观察组术后6 h、24 h、48 h、72 h的VAS疼痛评分均低于对照组,P<0.001;两组术后血清NE、Cor水平均较术前明显提高,P<0.001;但组间比较观察组术后的血清NE、Cor水平均低于对照组,P<0.001。结论细节优质护理的实施有利于促进腹腔镜阑尾切除术患者术后尽早康复,保持良好心态接受手术治疗,减轻患者的生理应激反应,促进术后康复治疗,值得推广。

Objective To explore the effect of detail quality nursing in laparoscopic appendectomy.Methods Two hundred and ten patients underwent laparoscopic appendectomy in our hospital from May 2018 to October 2019 were selected as the object,divided into random digital table method:the control group patients were given routine nursing measures,the observation group patients were given detail quality nursing on this basis,the incidence of postoperative complications,postoperative hospitalization time,nursing satisfaction,preoperative and postoperative psychological state score of the two groups were collected.Results The incidence of postoperative complications in the observation group was 3.81%,significantly lower than 11.43%in the control group,P<0.05;and the nursing satisfaction rate in the observation group was 97.14%,significantly higher than 88.57%in the control group,P<0.05.The SAS and SDS scores of patients in the observation group were lower than those in the control group 2 hours before operation and 24 hours after operation,P<0.001;The first postoperative exhaust time,defecation time,ambulation time,bowel sound recovery time and hospital stay time in the observation group were significantly shorter than those in the control group,P<0.001;The VAS pain score of the observation group was lower than that of the control group at 6 h,24 h,48 h and 72 h,P<0.001.The levels of serum NE and Cor in the two groups were significantly higher than those before operation,P<0.001;However,the levels of serum NE and Cor in the observation group were lower than those in the control group,P<0.001.Conclusion The implementation of detailed high-quality nursing is beneficial to promote the early recovery of patients after laparoscopic appendectomy,maintain a good attitude to accept surgical treatment,reduce the physiological stress response of patients,and promote postoperative rehabilitation,which is worth promoting.
