中药灌肠联合美沙拉嗪治疗大肠湿热型溃疡性直肠炎的临床效果

Clinical effect of self-made Chinese medicine enema formula combined with mesalazine in the treatment of ulcerative proctitis with damp-heat in the large intestine

目的研究中药灌肠联合美沙拉嗪治疗大肠湿热型溃疡性直肠炎的临床效果。方法将2020年4月至2021年4月于常德市第一中医医院肛肠科就诊的107例患者按照随机数学表法分为治疗组(n=53)及对照组(n=54)。对照组使用口服美沙拉嗪肠溶片,治疗组则是在对照组的基础上加用自拟中药方剂保留灌肠。观察比较两组患者的肠镜黏膜积分、临床症状积分、血清C反应蛋白水平以及总有效率。结果治疗组患者治疗后肠镜黏膜积分、临床症状积分、血清C反应蛋白水平均显著低于对照组,差异有统计学意义(P<0.05)。治疗组患者治疗后肠镜黏膜积分、临床症状积分、血清C反应蛋白水平均显著低于治疗前,差异有统计学意义(P<0.05)。治疗组患者的总有效率显著高于对照组,差异有统计学意义(P<0.05)。结论自拟中药灌肠方剂联合美沙拉嗪治疗大肠湿热型溃疡性直肠炎效果更佳,优势明显,值得推广应用。

Objective To study the clinical effect of Chinese medicine enema combined with mesalazine in the treatment of ulcerative proctitis.Methods A total of 107 patients admitted to the Anorectal Department of Changde First Hospital of Traditional Chinese Medicine from April 2020 to April 2021 were divided into treatment group and control group by random table method.The control group(n=54)was treated with oral mesalazine enteric-coated tablet,while the treatment group(n=53)was treated with retention enema of selfdesigned Chinese medicine prescription on the basis of the control group.Colonoscopic mucosal score,clinical symptom score,serum C-reactive protein level and total response rate were observed and compared between the two groups.Results The endoscopic mucosal score,clinical symptom score and level of serum C-reactive protein level after treatment in the treatment group were significantly lower than those in the control group,with statistically significant differences(P<0.05).The endoscopic mucosal score,clinical symptom score and inflammatory index of serum C-reactive protein level after treatment in the treatment group were significantly lower than those before treatment,with statistically significant differences(P<0.05).The total effective rate of the treatment group was significantly higher than that of the control group,with statistically significant difference(P<0.05).Conclusion The self-made Chinese medicine enema formula combined with mesalazine shows better effects and obvious advantages in the treatment of ulcerative proctitis with damp-heat in the large intestine,which is worthy of popularization and application.
