CICARE沟通模式在2型糖尿病患者延伸性护理中的应用效果

Application of CICARE communication mode in extended nursing care for patients with type 2 diabetes mellitus

目的评估实施六步标准沟通模式(CICARE)在2型糖尿病患者住院及出院延伸性护理工作中的效果。方法选取新疆医科大学第六附属医院2018年1月—2020年6月收治的2型糖尿病患者489例,采用随机数字表法分为对照组(244例)和观察组(245例)。对照组实施常规护理,观察组在此基础上运用CICARE沟通模式进行护理。比较两组患者入院时、出院时及出院第3、6个月的血糖值、总胆固醇、甘油三酯、护理满意度及生存质量情况。结果与对照组比较,观察组出院第3、6个月的空腹血糖值,餐后2 h血糖值,总胆固醇值和甘油三酯值降低,差异均有统计学意义(P<0.05);观察组出院第3、6个月总有效率(93.1%、86.5%)均高于对照组(81.2%、60.2%),且有效人数均显著增多(P<0.01);与对照组比较,观察组住院期间护理满意度显著提高(P<0.05);与对照组比较,观察组第3、6个月生理功能、心理和精神、治疗3个维度得分增加,差异均有统计学意义(P<0.05)。结论CICARE沟通模式护理既能提高住院期间2型糖尿病患者满意度及治疗效果,又能有效维持2型糖尿病患者出院后的治疗效果,提高患者自我健康管理能力及生存质量,值得临床推广应用。

Objective To evaluate the effect of implementing contact,introduction,communication,inquiry,answer,leave(CICARE)communication mode combined with WeChat in extended care for type 2 diabetes patients during hospitalization and discharge.Methods A total of 489 patients who were admitted to the hospital for the treatment of type 2 diabetes from January 2018 to June 2020 were selected and randomly divided into the control group(244 cases)and the observation group(245 cases)according to the random number table.The control group received normal routine nursing care,while the observation group received the nursing care with using CICARE communication mode.Two groups were compared in terms of blood glucose,blood fat control,nursing satisfaction and quality of life at 3rd and 6th month of admission and discharge.Results Compared with the control group,the fasting and 2h postprandial blood glucose levels,total cholesterol and triglyceride levels in the observation group at 3rd and 6th month after the discharge were decreased,and the differences were statistically significant(P<0.05).The total effective rates(93.1%and 86.5%)in the observation group at 3rd and 6th month after the discharge were higher than those in the control group(81.2%and 60.2%),and the number of effective persons was significantly increased(P<0.01);Compared with the control group,the satisfaction of the observation group with nursing was significantly improved.Compared with the control group,the scores of physiological function,psychological spirit and treatment in the observation group were increased with statistical significance(P<0.05).Conclusion The combination of CICARE model care with WeChat can not only improve satisfaction and treatment effect of patients during hospitalization,but also it effectively maintain the treatment effect of patients after discharge and improve patients'self-health management ability and quality of life,which is worthy of extensive clinical application.
