补阳还五汤治疗气虚血瘀型高血压脑出血临床观察

Clinical Observation on Buyang Huanwu Decoction in the Treatment of Hypertensive Cerebral Hemorrhage of Qi Deficiency and Blood Stasis Type

目的探讨补阳还五汤治疗高血压脑出血(气虚血瘀型)对中医证候积分、临床疗效的影响。方法以南阳市第一人民医院2020年2月—2021年2月收治74例高血压脑出血(气虚血瘀型)患者为例,随机分组。对照组37例,常规西医治疗;观察组37例,在常规西医治疗基础上给予补阳还五汤治疗,比较2组治疗前后中医证候积分变化、临床疗效、脑血管功能变化及药物不良反应发生率。结果治疗后,观察组中医证候积分低于对照组(P<0.05),临床总有效率(83.78%)高于对照组(62.17%)(P<0.05)。治疗后,观察组脑血管平均流速(21.02±1.70)cm/s高于对照组(17.90±1.46)cm/s(P<0.05),脑血管动态阻力(325.21±23.77)k Pa·s/m低于对照组(361.27±25.60)k Pa·s/m(P<0.05)。治疗期间,2组药物不良反应发生率差异无统计学意义(P>0.05)。结论在常规治疗基础上加以补阳还五汤,可降低高血压脑出血(气虚血瘀型)患者头目昏沉、肢体麻木等中医证候积分,提高临床疗效,并能改善患者脑血流状态,临床价值高。

Objective To investigate the effect of Buyang Huanwu decoction on traditional Chinese medicine(TCM)syndrome score and clinical effect of hypertensive cerebral hemorrhage(qi deficiency and blood stasis type).Methods 74 patients with hypertensive cerebral hemorrhage(qi deficiency and blood stasis type)were randomly divided into groups from February 2020 to February 2021.37 cases in the control group were treated by conventional western medicine;The observation group of 37 cases,on the basis of conventional western medicine treatment,was given Buyang Huanwu decoction.The TCM syndrome score changes,clinical efficacy,cerebrovascular function changes and adverse drug reactions rate were compared before and after the treatment in the two groups.Results After treatment,the TCM syndrome score of observation group was lower than that of control group(P<0.05),and the clinical effective rate of the observation group(83.78%)was higher than that of control group(62.17%)(P<0.05).After treatment,the mean velocity of cerebral vessels in the observation group(21.02±1.70)cm/s was higher than that of the control group(17.90±1.46)cm/s.The dynamic resistance of cerebral vessels(325.21±23.77)k Paos/m was lower than that of the control group(361.27±25.60)k Paos/m(P<0.05).During the treatment,there was no significant difference in the incidence of adverse drug reactions between the two groups(P>0.05).Conclusion On the basis of routine treatment,Buyang Huanwu decoction can reduce the TCM syndrome score of head depression and numbness of limbs in patients with hypertensive cerebral hemorrhage(qi deficiency and blood stasis type),and improve the clinical effect.It can improve the blood flow of the patients,and has high clinical value.
