广东省药学会推动的药学门诊正式列入国家卫生健康委员会服务规范

Pharmacist-managed Clinics Promoted by Guangdong Pharmaceutical Association were Officially Written into the Service Standards of China National Health Commission

2021年10月9日,国家卫生健康委正式将药学门诊列入服务规范。广东省药学会(以下简称“本会”)于2017年2月和4月两次发文,特别是4月的《关于推进药学门诊工作的通知》(粤药会[2017]26号),呼吁医院药师积极开设以药物治疗管理服务(medication therapy management services,MTMs)为工作模式的收费的药学门诊,在现有的医疗收费项目中寻找与药学服务相关的收费,体现药学服务的价值。为体现与医师门诊不同的特点,本会2017年从第22届欧洲医院药学大会引入“处方精简”理念,并呼吁药学门诊应开展药师主导的处方精简工作。为提高用药的精准度,本会呼吁药师进行与用药相关的医学检验。通过合作药物治疗管理(Collaborative Drug Therapy Management,CDTM)模式,与医师达成协议进行处方调整以解决药师没有处方权的问题。2018年8月20日,本会发布了《药学门诊试行标准》,还推进了药学门诊的专科化,如老年慢病、抗栓(凝)、妊娠哺乳期、疼痛、肿瘤、移植免疫抑制剂等药学门诊。

On October 9,2021,Pharmacist-managed clinics were officially written into the service standards of China National Health Commission.In February and April 2017,the Guangdong Pharmaceutical Association(GDPA) twice appealed to hospital pharmacists to open clinics using the medication therapy management services(MTMs) model and get the payment from all potential sources of income.The GDPA introduced the concept of "deprescribing" from the 22nd Congress of the European Association of Hospital Pharmacists in 2017,and called for pharmacists implementing deprescribing in pharmacist-managed clinics(PMCs).To improve the accuracy of medication,the GDPA calls on pharmacists to conduct medical tests related to medication.As pharmacists have no right to prescribe,the GDPA appealed to pharmacists to obtain the drug-prescription authority by signing the collaborative drug therapy management(CDTM) agreements with physicians.On August 20,2018,the GDPA issued the Standard for Pharmacist-managed Clinics.The GDPA also tried to differentiate PMCs into diverse areas,such as for chronic diseases of the elderly,antithrombotic(coagulation),pregnancy and lactation,pain,tumor,transplantation immunosuppressive agents.
