摘要
目的系统评价2型糖尿病患者发生轻度认知功能障碍的影响因素。方法计算机检索PubMed、Web of Science、EMbase、The Cochrane Library、CBM、WanFang Data、VIP和CNKI数据库,搜集2型糖尿病患者发生轻度认知功能障碍影响因素的研究,检索时限均从建库至2021年12月31日。由2名研究者独立筛选文献、提取资料并评价纳入研究的偏倚风险后,采用Rev Man 5.4软件和Stata 12.0软件进行统计分析。结果共纳入32个研究,包括7519例研究对象。Meta分析结果显示:2型糖尿病患者发生轻度认知功能障碍的主要影响因素包括年龄、糖尿病病程、受教育程度、脑梗塞、高血压、吸烟、胰岛素抵抗指数、糖化血红蛋白、同型半胱氨酸水平。结论当前证据显示,年龄、病程、受教育程度等是2型糖尿病患者发生轻度认知功能障碍的影响因素。受纳入研究数量和质量的限制,上述结论尚待更多高质量研究予以验证。
Objective To systematically review the influencing factors of mild cognitive impairment in type 2 diabetic patients.Methods PubMed,Web of Science,EMbase,The Cochrane Library,CNKI,WanFang Data,VIP,and CBM databases were electronically searched to collect studies on the influencing factors of mild cognitive impairment in patients with type 2 diabetes from inception to December 31,2021.Two reviewers independently screened the literature,extracted data,and assessed the risk of bias of included studies;then,meta-analysis was performed by using RevMan 5.4 software and Stata 12.0 software.Results A total of 32 studies involving 7519 subjects were included.The results of the meta-analysis showed that the main influencing factors of mild cognitive impairment in type 2 diabetic patients were age,duration of type 2 diabetes,educational level,cerebral infarction,hypertension,smoking,insulin resistance index,glycosylated hemoglobin,and homocysteine.Conclusion Current evidence shows that some factors such as age,duration,and educational level are the main influencing factors of mild cognitive impairment in type 2 diabetic patients.Due to the limited quality and quantity of the included studies,more high quality studies are needed to verify the above conclusions.
