针刀治疗冻结肩的临床疗效观察

Observation On the Clinical Effect of Needle-knife for Frozen Shoulder

目的:观察针刀治疗冻结肩的临床疗效,探索冻结肩的有效治疗方法。方法:选取2020年8月~2021年3月前来我院就诊的冻结肩患者58例,随机分为针/组和对照组各29例。针刀组采用针/治疗,对照组采用药物治疗,记录两组患者治疗前后疼痛视觉模拟评分法(VAS)评分、肩关节活动范围(ROM)评分和肩关节疼痛及功能指数(SPADI)评分。结果:治疗后,两组患者的VAS评分均较治疗前降低(P<0.01),且针刀组治疗后的VAS评分低于对照组(P<0.01)。治疗后,两组患者的前屈、后伸、外展活动范围均较治疗前增大(P<0.01),且针刀组治疗后的前屈、后伸、外展活动范围显著大于对照组(P<0.01)。治疗后,两组患者的疼痛评分、功能障碍评分、SPADI评分均较治疗前降低(P<0.01),且针刀组治疗后的疼痛评分、功能障碍评分、SPADI评分显著X于对照组(P<0.01)。结论:针刀治疗能有效缓解冻结肩患者的临床症状,改善其肩关节活动范围及功能障碍情况,可作为治疗冻结肩的有效方法。

Objective:To observe the clinical efficacy of needle-knife on frozen shoulder and explore effective treatment methods for frozen shoulder.Method:58 patients with frozen shoulder who came to our hospital from August 2020 to March 2021 were randomly divided into needle knife group and control group.The Acupotomy group was treated with Acupotomy and the control group was treated with drugs.The pain visual analogue scale(VAS) score,shoulder range of motion(ROM) score and shoulder pain and function index(SPADI) score were recorded before and after treatment.Results:after treatment,the VAS scores of the two groups were lower than those before treatment(P<0.01),and the VAS scores of the needle knife group were lower than those of the control group(P<0.01).After treatment,the range of flexion,extension and abduction in the two groups increased compared with that before treatment(P<0.01),and the range of flexion,extension and abduction in the needle knife group was significantly greater than that in the control group(P<0.01).After treatment,the pain score,dysfunction score and SPADI score of the two groups were lower than those before treatment(P<0.01),and the pain score,dysfunction score and SPADI score of the needle knife group were significantly higher than those of the control group(P<0.01).Conclusion:Needle knife treatment can effectively alleviate the clinical symptoms of patients with frozen shoulder,improve the range of motion and dysfunction of shoulder joint,and can be used as an effective method for the treatment of frozen shoulder.
