摘要 目的:通过对骨盆-下肢运动功能变化的量化分析,探讨骶髂关节前错位患者的步态改变。方法:选取2021年3月~2022年2月在本院诊治的骶髂关节前错位患者40例,均为右侧病变(骶髂关节前错位组),健康志愿者40人(正常人组)纳入本研究。采用DIERS pedogait步态分析系统采集所有受试者的步态数据,包括单足支撑相、摆动相和足夹角,并对骶髂关节前错位患者进行Oswestry功能障碍指数(oswestry disability index,ODI)评分。采用SPSS24.0软件对所有受试者的步态数据进行统计学分析,比较两组步态参数的差异,并分析ODI量表评分与骶髂关节错位患者步态参数之间的相关性。结果:与对照组同侧肢体相比较,研究组患侧肢体的单足支撑相减少、摆动相增加、足夹角角度减小,结果差异有统计学意义(P<0.05);通过相关性分析发现:ODI评分与骶髂关节前错位组患侧的单足支撑相、摆动相有相关性(P<0.05),与足夹角无明显相关(P>0.05)。结论:本研究发现,骶髂关节前错位患者步态与正常人比较具有差异,表现为患侧单足支撑相缩短、摆动相延长、足夹角减小。患侧单支撑相、摆动相与ODI评分具有相关性,表明患者疼痛越明显,功能越差,步态异常越明显。 Objective:Through the analysis of the changes of pelvic-lower limb motor function to discusses the gait changes of patients with anterior dislocation of sacroiliac joint.Method:Forty patients with right anterior sacroiliac dislocation diagnosed(anterior sacroiliac dislocation group),who treated in our hospital from March 2021 to February 2022 were selected,and 40 healthy volunteers(normal group) were included in this study.DIERS pedogait gait analysis system was used to collect gait data of all subjects,including single foot support phase,swing phase and foot angle.The patients with anterior dislocation of sacroiliac joint were scored by Oswestry disability index(ODI).Adopt SPSS 24.0 to statistically analyze the gait data of all subj ects,compare the differences of gait parameters between the two groups,and analyze the correlation between ODI and gait parameters of patients.Result:Compared with the ipsilateral limb of the control group,the single foot support phase,swing phase and foot angle of the affected limb in the study group decreased,and the difference was statistically significant(P<0.05);Correlation analysis:In anterior sacroiliac dislocation group,ODI score was correlated with the single foot support phase and swing phase of the affected side(P<0.05),but not with the foot angle(P>0.05).Conclusion:This study found that the gait of patients with anterior dislocation of sacroiliac joint was different from that of normal people,which showed that the support phase of one foot on the affected side was shortened,the swing phase was prolonged,and the foot angle was reduced.The single support phase and swing phase of the affected side were correlated with ODI,indicating that the more obvious the pain,the worse the function and the more obvious the abnormal gait.
