腹腔镜联合胆道镜保胆取石术治疗胆囊结石的效果及对应激指标、NRS评分的影响

Effect of Laparoscopy Combined with Choledochoscope for Gallbladder-preserving Cholecystolithotomy in the treatment of Gallbladder Stones and Its Effects on Stress Indicators and NRS Score

目的分析腹腔镜联合胆道镜保胆取石术在胆囊结石患者中应用效果及对应激指标、疼痛的影响。方法选取2018年1月—2021年10月收治的胆囊结石83例,按照治疗方式分为研究组42例和对照组41例。研究组采用腹腔镜联合胆道镜保胆取石术,对照组采用腹腔镜胆囊切除术。比较2组手术相关指标、应激反应、疼痛情况及并发症发生情况。结果研究组手术时间长于对照组,术中出血量少于对照组,肠道功能恢复时间短于对照组(P<0.01)。研究组术后皮质醇、C反应蛋白、血糖水平、数字疼痛强度量表评分及并发症发生率低于对照组(P<0.05)。结论对胆囊结石患者予以腹腔镜联合胆道镜保胆取石术效果更显著,能有效减少术中出血量,减轻术后疼痛及应激反应,且安全性较高。

Objective To analyze the application effect of laparoscopy combined with choledochoscopy for gallbladder-preserving cholecystolithotomy in patients with gallbladder stones and its effects on stress indicators and pain.Methods In total,83 patients with gallbladder stone treated from January 2018 to October 2021 were selected and divided into the research group(n=42)and the control group(n=41)according to the treatment method.The research group was treated with laparoscopy combined with choledochoscopy for gallbladder-preserving cholecystolithotomy,and the control group was treated with laparoscopic cholecystectomy.The operation related indexes,stress response,pain and complications were compared between the two groups.Results The duration of operation in the research group was longer than that in the control group,the amount of intraoperative bleeding was less than that in the control group,and the recovery time of intestinal function was shorter than that in the control group(P<0.01).Cortisol,C-reactive protein and blood glucose,Numerical Rating Scale scores and incidence of complications were lower in the research group than those in the control group(P<0.05).Conclusion Laparoscopy combined with choledochoscopy for gallbladder-preserving cholecystolithotomy is more effective in patients with gallbladder stones,which can effectively reduce the amount of intraoperative bleeding,reduce the postoperative pain and stress response with good safety.
