甲泼尼龙琥珀酸钠治疗低频下降型SHL患者的效果及对听阈水平、血流动力学的影响

Effect of methylprednisolone sodium succinate on patients with low-frequency descending SHL and its influence on the level of hearing threshold and hemodynamics

目的:分析低频下降型突发性耳聋(SHL)患者应用甲泼尼龙琥珀酸钠治疗的效果及对血流动力学、听阈水平的影响。方法:选取87例低频下降型SHL患者作为研究对象,采用随机数字表法分为对照组43例和观察组44例。对照组43例给予地塞米松磷酸钠于耳后沟行乳突骨膜下注射治疗,观察组44例给予甲泼尼龙琥珀酸钠行耳后沟乳突骨膜下注射治疗。比较两组患者症状改善情况、听阈水平、血流动力学指标。结果:治疗后,观察组患者耳闷胀感、耳鸣评分低于对照组(P<0.05);观察组纯音听阈值及受损频率平均听阈值均低于对照组(P<0.05);观察组全血中高切还原黏度、PV、TK及全血低切还原黏度均低于对照组(P<0.05)。结论:低频下降型SHL患者应用耳后沟乳突骨膜下注射甲泼尼龙琥珀酸钠治疗,效果显著,能有效减轻耳鸣、耳闷胀感症状,增加内耳血流量,有利于促进听觉功能的恢复。

Objective:To analyze the effect of subperiosteal injection of methylprednisolone into the mastoid process of posterior auricular sulcus on patients with sudden hearing loss(SHL)and on hemodynamics and hearing threshold.Methods:87 SHL patients were selected as the research objects and the randomized into 2 groups,with 43 cases in the control group and 44 cases in the observation group.The control group were given subperiosteal injection of dexamethasone sodium phosphate into the mastoid process of posterior auricular sulcus,while the observation group were given subperiosteal injection of methylprednisolone sodium succinate into the mastoid process of posterior auricular sulcus.The improvement of symptoms,hearing threshold level and hemodynamic indicators were compared between groups.Results:After treatment,the scores of ear fullness and tinnitus in the observation group were lower than those in the control group(P<0.05);the pure tone hearing threshold and the average impaired frequencies in the observation group were lower than those in the control group(P<0.05);the whole blood medium and high-shear reduction viscosity,PV,TK and whole blood low-shear reduction viscosity were lower than those of the control group(P<0.05).Conclusion:For patients with low-frequency descending SHL,the treatment effect of subperiosteal injection of methylprednisolone sodium succinate into the mastoid process of posterior auricular sulcus can effectively reduce the symptoms of tinnitus and ear fullness,increase the blood flow of the inner ear,and promote the recovery of auditory function.
